PATOLOGÍA
• La hemorragia ocurre principalmente dentro de la
red capilar, la cual se comunica libremente con el
sistema venoso, aunque el sangrado también puede
ser arterial.

EPIDEMIOLOGÍA
• Incidencia ↑ con menor edad gestacional y peso al nacer.
• Más frecuente en prematuros < 32 semanas y ˂ 1500 g.
• NICHD 2003 – 2010, 9575 prematuros
– RN entre 22 y 28 semanas, 401 – 1500 g
– Incidencia de HIV: 36%
– Prevalencia de HIV severa aumentó con menor edad gestacional:
• 38% (22 sem), 7% (28 sem)
• Estudios previos sugerían que RN con MBPN que eran PEG
tenían menor riesgo de HIV, pero estudios posteriores
mostraron que no existe diferencia significativa
• Cohorte en < 32 semanas: 21.6% en PEG y 23.9% en AEG.

FACTORES DE RIESGO

• Síntomas no específicos: Alteración del nivel de conciencia, hipotonía, alteración de
movimientos, cambios en posición o movimiento de los ojos, alteración del patrón
respiratorio
– Deterioro catastrófico
• Menos común
• Evolución en minutos a horas
• Signos: Estupor/coma, hipoventilación/apnea, postura de decerebración, convulsiones
generalizadas (especialmente tónicas), flacidez, anormalidades en pares cranales (pupilas fijas
a la luz), hipotensión, bradicardia, acidosis metabólica, etc.

CLÍNICA
• Todas las HIV suelen ocurrir en los primeros 5 días
de vida.
– 1er día → 50%
– 2do día → 25%
– 3er día → 15%
– 4to día o después→ 10%
• Todos RN ˂ 30 semanas
• Primera US → 7 a 14 días de vida
• Repetir a las 36 a 40 semanas de edad postmenstrual
• Considerar si signos clínicos de deterioro, enfermedad
grave o alto riesgo de sangrado
• En RN 30 – 34 semanas: si sospecha clínica

MANEJO
Medidas generales
– Mantener adecuada perfusión arterial
• Evitar hipotensión o hipertensión
• Preservar adecuado flujo cerebral
– Mantener adecuada ventilación y oxigenación
• Evitar hipocapnea, hipercapnea o acidosis
– Soporte hídrico, metabólico y nutricional
adecuado.

• Detención espontánea sin necesidad de intervención (40%)
• Progresión rápida (10%)
