1. DEFINICIÓN:
CIE10 P210
La asfixia perinatal es la condición en la cual se presenta una alteración grave en el
intercambio gaseoso del recién nacido como consecuencia de diferentes noxas que
ocurren antes (intrauterino), durante, en los primeros minutos posteriores al
nacimiento y hasta los 28 días de nacido.
2. SUBJETIVO
No aplica.
4. ALCANCE
Todos los bebés que nacen en cualquiera de los ambientes del Hospital General
Enrique Garcés o aquellos que eventualmente llegan transferidos con el diagnóstico
presuntivo o confirmado de asfixia perinatal.
5. RESPONSABLES
Responsable de la Supervisión:
Coordinador Médico de la Unidad.
Ejecutora y Supervisora de Enfermería de la Unidad de Neonatología.
Responsable de la Aplicación:
Personal de Médicos Tratantes.

CANDIDATOS:
1) Recién nacidos postérmino, a término o prematuros tardíos cuya edad
gestacional se haya estimado por la fecha de la última menstruación (FUM) o por
métodos indirectos.
2) Aunque puede ser difícil establecer la presencia, el origen y el tiempo de inicio de
la hipoxia fetal, en todo embarazo es conveniente tener en cuenta (y estar alerta
ante) los siguientes factores de riesgo que podrían generar isquemia/hipoxia fetal
y conducir a asfixia perinatal:
a) Edad materna ≥ 35 años, antecedentes familiares de convulsiones o
enfermedades neurológicas y tratamiento de infertilidad;
b) Enfermedad tiroidea materna, diabetes, preeclampsia grave, gestación
múltiple, anomalías genéticas, restricción del crecimiento fetal, traumatismo
materno y hemorragia anteparto;
c) Desprendimiento de la placenta, prolapso del cordón umbilical, ruptura uterina,
hemorragia feto/materna y distocia de hombros, así como fiebre materna con
o sin corioamnionitis; y,
d) Lesión o trauma natal y posnatal, insuficiencia cardiorrespiratoria inmediata,
evidencia de cardiopatía congénita y anomalías neurológicas.

REQUERIMIENTOS:
• Tres (3) ampollas de 10 ml de sulfato de magnesio al 20% (1 cc = 200 mg).
• Una ampolla de aminofilina de 250 mg en 10 mL.
• Cinco (5) a catorce (14) ampollas de eritropoyetina de 2000 UI/2 mL.
• Un (1) termómetro rectal individual para cada niño.
• Un ventilador mecánico disponible para el niño en cuestión.
• Un monitor neurológico cerebral (electroencefalograma de amplitud integrada).
• Disponibilidad para realizar determinaciones seriadas de electrólitos y de calcio y
magnesio séricos.
CONDUCTA:
1) Hidratación: aporte de líquidos y electrolitos de acuerdo a los requerimientos
basales habituales para el peso y la edad gestacional.
2) Hematología: mantenimiento del hematocrito por debajo de 65% y por encima de
40%.
