1. DEFINICIÓN:

La hipoglucemia se define a un nivel de glucosa en sangre menor de 40 mg/dl en un
recién nacido/a pretérmino o a término en las primeras 72 horas de vida y
posteriormente niveles menores de 50 mg/dl.
Es usualmente transitoria y afecta a recién nacidos quienes son grandes o pequeños
para edad gestacional, prematuros, o hijos de madres diabéticas.
También como consecuencia de stress sistémico como sepsis, asfixia, distress
respiratorio, policitemia, hipotermia.
La hipoglucemia persistente o recurrente usualmente indica un hiperinsulinismo
congénito o un trastorno metabólico.
La forma transitoria se produce por una disminución de la producción, aumento de
la utilización de glucosa o por hiperinsulinismo transitorio, debido a que la
alimentación se retrasa de 4 a 6 horas luego del nacimiento. Por lo general cede con
la administración precoz de la alimentación.
La hipoglucemia transitoria responde rápidamente al tratamiento con cargas de < de
12 mg/kg/min y requiere un aporte continuo por menos de 7 días.
La forma persistente necesita de cargas de > de 12 mg/kg/minuto y/o un aporte
continuo por más de 7 días.

• Alcohol al 70%.
• Estetoscopio.
• Bomba de infusión.
• Tubos para toma de muestra de sangre de tipo pediátrico tapas lila y roja.
• Agujas hipodérmicas de 23 G.
• Guantes estériles.
• Tubos capilares.
• Plastilina.
• Centrifugadora.
• Medicación (hidrocortisona/diazóxido).

HIPOGLUCEMIA SINTOMÁTICA:
A TENER EN CUENTA:
1. En los recién nacidos de término alimentados con pecho materno, las concentraciones
de glucosa son más bajas que en los alimentados con fórmulas. Estos bebés toleran
niveles bajos de glucosa durante el primer día de vida entre 30–35 mg/dl, sin presentar
manifestaciones clínicas ni secuelas.
2. Las tiras reactivas miden la glucosa en sangre total que es un 15% inferior a la de los
niveles plasmáticos.
3. Las tiras reactivas están expuestas a resultados falsos positivos y falsos negativos.
4. Para establecer el diagnóstico de hipoglucemia, es necesaria la confirmación de la
glucosa en el laboratorio (glucosa plasmática).
5. Realizar valoración con endocrinología con los exámenes realizados.
