El término Infección aguda de las vías
respiratorias superiores (IAVRS) se refiere a
la enfermedad infecciosa, que afecta al
aparato respiratorio desde la nariz hasta antes
de la epiglotis, durante un periodo menor a
15 días,
 Introducción
La infección respiratoria aguda (IRA) se considera como una de las principales
causas de morbilidad y mortalidad en el mundo, pues, en promedio, cada año
suceden 3.460.000 muertes por esta enfermedad, que afectan a todas las
poblaciones del mundo.
El grupo poblacional con mayor riesgo de morir por IRA son los menores de 4 a 5
años, las personas que viven con alguna enfermedad crónica de base, en
especial aquellos que reciben tratamiento con algún tipo de inmunosupresión.
 Incidencia .
La infección aguda de vías respiratorias superiores (IAVRS) es la primera causa de enfermedad,
en México.

Dos terceras partes de los casos observados en la población general ocurren en

pacientes pediátricos; aproximadamente una tercera parte suceden en menores a 4
años de edad.

.En el 80 a 90% la etiología es viral (ENSANUD 2012); estos episodios de infección

son generalmente benignos y se auto limitan en corto tiempo

En una proporción menor, entre 15 hasta 30% de los casos en niños y entre 5 hasta
20% en adultos, la etiología es bacteriana.

• Medidas como elevación de la cabecera de la cama y aspiración de

secreción nasal con una perilla pueden promover bienestar.
• El reposo que necesita el paciente depende directamente de cuan
enfermo se siente; no hay contraindicación para la actividad física
normal si el paciente está afebril y con buen estado general.
• Irrigaciones salinas nasales.
• Los vapores inhalados por frotación de ungüento puedes favorecer el
estado de salud del paciente.
 Tratamiento farmacológico
. • La prescripción de Ibuprofeno Y acetaminofén,
son recomendables para el alivio sintomático de la
faringitis aguda.
• Los antibióticos prescritos en forma inmediata
favorecen una discreta diferencia en la
satisfacción del paciente o el cuidador,
comparado con el grupo
• de pacientes con prescripción retrasada (92%
versus 87%.
• Aconsejar a los padres o cuidadores el uso de
paracetamol para el manejo de la fiebre y el dolor
en el niño con faringitis aguda.
Prevención primaria
Las medidas para prevenir y disminuir el contagios de IAVAS son:

• Lavado de manos frecuente con agua y jabón.

• Evitar que los niños con resfriado común compartan con otros juguetes o
chupones y lavarlos con agua y jabón cuantas veces sean posibles.
• Invitar a los visitantes a lavarse las manos antes de cargar a los niños.
• Evitar el contactos con personas que están cursando con enfermedades
respiratorias agudas.
• Promover la lactancia materna exclusiva durante los primeros 6 meses de
edad y continuar con la leche materna hasta los 12 meses.
• Evitar que los niños asistan a guarderías cuando presenten IAVAS.
VIGILANCIA Y SEGUIMIENTO
Se le recomienda a los padres de los pacientes con IAVAS que deben acudir a
revaloraciones cuando:

• La fiebre persista durante mas de tres días.

• Empeoren los síntomas o aparezcan nuevos después de 3 a 5 días de evolución.
• No presente mejorías de los síntomas después de 7 a 10 días.

Prevención siempre será el mejor tratamiento de infecciones respiratorias pueden ser
prevenidas o mejorar su diagnóstico con varias medidas:
• Mejorar la nutrición de la infancia y tener hábitos saludables durante la vida del
adulto.
• Promover la lactancia materna, con el fin de reforzar el sistema inmune de los
infantes.
• Asegurar una inmunización completa.
• Mejorar las condiciones de vida para evitar el hacinamiento.
• Evitar la exposición al humo del tabaco.
• Reducir la contaminación del aire en interiores.
• Tratar o prevenir la infección del VIH.
• Administrar antibióticos profilácticos en niños inmunosuprimidos.
• Prevenir la transmisión del VIH de madre a hijo.
• La vacunación es uno de los mayores logros de la ciencia médica, y es la manera
más eficaz para prevenir este tipo de enfermedades y de disminuir las tasas de
mortalidad. Los países con tasas de inmunización más bajas representan más de dos
tercios de la carga de la enfermedad prevenible por vacuna.
