Afección crónica que afecta la manera en la que el cuerpo procesa el azúcar en sangre
(glucosa).
En la diabetes tipo 2, el cuerpo de la persona no produce suficiente insulina o es
resistente a la insulina.
2. Factores relacionados
Tiene prediabetes.
Tiene sobrepeso.
Tiene 45 años o más.
Uno de sus padres o uno de sus hermanos o hermanas tiene diabetes tipo 2.
Hace actividad física menos de tres veces a la semana.
Ha tenido alguna vez diabetes gestacional (diabetes durante el embarazo) o su
bebé pesó más de 9 libras al nacer.
Es afroamericano, hispano o latino, indoamericano o nativo de Alaska. (Algunos
nativos de las islas del Pacífico y asiático americanos también tienen un riesgo
mayor).
Puede prevenir o retrasar la diabetes tipo 2 con cambios simples de estilo de vida de
eficacia comprobada, como bajar de peso si tiene sobrepeso, alimentarse más
saludablemente y hacer actividad física regularmente.
1. Síntomas
En la población en general
Las acciones de prevención primaria deben ejecutarse, no sólo a través de
actividades médicas, sino también con la participación y compromiso de la comunidad y
autoridades sanitarias utilizando los medios de comunicación masivos existentes en
cada región como radio, prensa y televisión, entre otros.
Los factores protectores para la prevención y control de esta enfermedad consisten
en modificar los cambios en el estilo de vida que abarca reducción de peso, una
adecuada nutrición, la realización de ejercicio y la disminución de los factores de riesgo
cardiovascular.
La intervención inicial y a lo largo del padecimiento se realizará especialmente
con tratamiento no farmacológico y consistirá en:
● Educación para la salud: folletos, revistas y boletines, entre otros.
● Promoción de la salud: corrección de factores dentro del estilo de vida.
● Prevención y corrección de obesidad: dietas con bajo contenido graso y
azúcares refinados y alta proporción de fibra alimentaria.
● Uso racional y prescripción adecuada de medicamentos diabetogénicos, por
ejemplo, diuréticos, corticoides, beta-bloqueadores.
● Promoción del ejercicio físico rutinario y programado.
● Integración a grupos de ayuda mutua, encaminados a que el o la paciente sea
responsable de su autocuidado y permanentemente se autogestión en pro de su
control metabólico.
Prevención secundaria.
Estará encaminada a pacientes ya confirmados con diabetes mellitus y cuyos objetivos
son evitar la aparición de complicaciones agudas, y evitar o retrasar las complicaciones
crónicas.
Las acciones para cumplir los objetivos propuestos se fundamentan en el control
metabólico óptimo y permanente de la enfermedad.
Prevención Terciaria
Estará dirigida a pacientes que presentan complicaciones crónicas y tiene como
objetivo evitar la discapacidad por insuficiencia renal, ceguera, pie diabético y evitar la
mortalidad temprana por enfermedad cardiovascular.
Estas acciones requieren de la participación de profesionales especializados en las
diferentes complicaciones.
● Control de peso
● El control de peso es una de las metas básicas para la prevención de la
diabetes.
● La reducción ideal de peso debe ser entre el 5% y el 10% del peso total. Debe
ser una meta anual hasta alcanzar su peso ideal.
El primer paso en el tratamiento de la diabetes en adultos mayores debe ser la
instauración de un régimen alimentario que lo conlleve a una disminución de peso,
sobre todo si presentan sobrepeso, para que de esta manera disminuya el patrón de
insulino resistencia, sin provocar pérdida de masa muscular (sarcopenia). Es
importante tener en cuenta la edad del paciente, comorbilidades asociadas o no a la
diabetes mellitus, nivel de funcionalidad y soporte social.
3. Control
Existen varias personas con diabetes tipo 2 que pueden controlar sus concentraciones
de glucosa en la sangre con tan solo dieta y ejercicio, otras pueden necesitar pastillas
para la diabetes o inyecciones de insulina, junto con medicamentos para controlar otras
afecciones
Una vez que se ha diagnosticado la diabetes, se procede a crear un plan de control de
la enfermedad. El plan se basará en el estilo de vida, preferencias, objetivos de salud y
otras afecciones de salud que el paciente tenga.
Este plan de control le indicará al paciente como: mantener un registro de las
concentraciones de glucosa, elegir alimentos saludables, mantenerse activo y tomar
sus medicamentos.
4. Tratamiento
El tratamiento de la DM tiene como propósito aliviar los
síntomas, prevenir las complicaciones agudas y crónicas,
mejorar la calidad de vida y reducir la mortalidad por esta
enfermedad o por sus complicaciones. El tratamiento debe ser
adaptado a las características y necesidades del paciente.
El tratamiento se divide en farmacológico y no farmacológico.

Plan Alimenticio
El aspecto nutricional es la herramienta fundamental para el tratamiento del paciente
diabético; por ello, el plan de alimentación debe considerar las características de cada
paciente de acuerdo con sus costumbres y preferencias en cuanto a los alimentos.
El objetivo principal del tratamiento es hacer los cambios de alimentación necesarios,
que favorezcan el control de la enfermedad y ayuden a prevenir complicaciones,
manteniendo en valores normales los niveles de glucosa en sangre.
Nota: Para prevenir el aumento o
disminución de azúcar en sangre, es
importante la constancia en los horarios,
tanto para las comidas, como para las
colaciones.
Llene la mitad con verduras sin almidón, como ensalada, ejotes verdes, brócoli,
coliflor, repollo y zanahorias.
Llene un cuarto del plato con una proteína magra, como pollo, pavo, frijoles, tofu
o huevos.
Llene un cuarto con carbohidratos. Los alimentos con mayor contenido de
carbohidratos incluyen los granos, las verduras con almidón (como las papas), el
arroz, la pasta, los frijoles, las arvejas, las frutas y el yogur. Una taza de leche
también cuenta como alimento con carbohidratos.
Luego acompañe la comida con agua o una bebida de bajas calorías, como té
frío sin azúcar.
En la actualidad, las porciones que sirven en los restaurantes son bastante más
grandes de lo que eran hace algunos años. ¡Un plato principal puede equivaler a 3 o 4
raciones! Los estudios realizados muestran que las personas tienden a comer más
cuando les sirven más comida; por eso, es realmente importante mantener el control de
las porciones para manejar el peso y el nivel de azúcar en la sangre.

o caja. A la hora de la cena, reduzca la tentación de servirse de nuevo, dejando los

be tailored to the characteristics and needs of the patient.

the control of the disease and help prevent complications, maintaining normal blood
glucose levels.
