Quemaduras
Una quemadura es una alteración de la
integridad de la piel y las capas que la
conforman.
Depende del mecanismo de lesión, del
agente, la extensión, la profundidad. Puede
ocasionar una disfuncionalidad incluso con
alteración permanente.
➔ Más del 80% de las lesiones térmicas
son prevenibles.
➔ Mortalidad alta en niños: grado de
afectación sistémica y tipo de
tratamiento.
Diferencias del niño y adulto
★ La vía aérea de los niños es más vulnerable: mayor
probabilidad de obstrucción de la vía aérea.

➔ Quemadura por combustión: intubar


➔ Lesiones a nivel de narinas o pilares amigdalinos.
➔ Compromiso de VAI: disnea progresiva.

★ Los niños son más susceptibles a la intoxicación por CO


★ Los niños tienen mayor predisposición a hipotermia, por su
mayor ASC.
★ Los niños tienen diferentes proporciones en cuanto a ASC lo
que puede afectar el cálculo de la extensión de la
quemadura.
★ La piel del niño es de menor espesor que la del adulto
(lesiones más profundas y graves)

- Tercer día: 3000 mL x ASC quemada (líquidos de sustitución) + calculados en las
2000 mL x ASC (líquidos de mantenimiento). Empezar la primeras 8h desde
administración de potasio: 30 mEq/ ASC/día la quemadura el
resto en 16h
Manejo hospitalario
3. Analgesia
Primera medida es elevar la extremidad por encima del corazón para disminuir
edema.
El tratamiento farmacológico se pone hasta la fase de epitelización, las opciones son:

1. Sulfato de morfina 0.1 - 0.2 mg/kg IV cada 4 a 6


horas → Quemaduras graves
2. Fentanilo 1 - 2 picograma/kg intermitente cada 1 a 2
horas o infusion continua de 1 a 5 picogramo/ kg/ hr
→ Vigilar depresion respiratoria
3. Ketorolac 0,5 mg/kg cada 6 horas
4. Ketamina para el cambio de vendaje → Vigilar
alucinaciones (disociativas) (se asocia a midazolam)
Si es una quemadura de pequeña extensión se puede iniciar con
ibuprofeno, con hidratación oral, con manejo ambulatorio

4. Toxoide tetánico

● Heridas no superficiales y niños no


vacunados
● Niños vacunados con última dosis >5-10
años
● Esquema incompleto (no tiene 3 dosis)

5. Considere coloides

Se colocan a las 24 horas y solo cuando se tiene más de


30% de ASC quemada. La dosis dependerá del ASC
quemada

- 30-50%: 0,3 mL/kg/ASC quemada/24 horas


- 50-70%: 0,4 mL/kg/ASC quemada/24 horas
- 70-100%: 0,5 mL/kg/ASC quemada/24 horas.
