Síndrome antifosfolipídico

El SAF neonatal es inusitado, hay que tener en cuenta que los AAF adquiridos
pasivamente desaparecen por completo a los 6-12 meses de edad (Motta M,
2006), y el pediátrico (SAF-p) (edad media de iniciación a los 10 años) muy
raro, 3% de los casos y sólo ligeramente más común en el sexo femenino
1,2:1. Los niños tienen un riesgo de trombosis recurrentes aún mayor que los
adultos, sobre todo arteriales (Lirola MJ, 2014; Wincup C, 2018).
Patogenia y anticuerpos
Los AAF principales son: anticoagulante lúpico (AL), que se detecta por
interferencia con las pruebas de coagulación fosfolípido dependientes,
anticardiolipina (aCL o aCA) de tipo inmunoglobulina (Ig) G y/o M y antiβ2-
Glucoproteína I (Antiβ2-GP1) IgG y/o IgM, que se detectan mediante pruebas
inmunológicas (ELISA).
Valores para los AAF: aCL (positivos bajos: entre 20-39,9 unidades GPL o
MPL; positivos medios: 40-79,9 unidades GPL o MPL; y, positivos altos: >80
unidades GPL o MPL) y Antiβ2-GPI (positivos: más de 20 unidades GPL o
MPL). GPL, del inglés IgG phospholipids y MPL, del inglés IgM
phospholipids. 1 unidad GPL o MPL es igual a 1µ de AAF IgG o IgM.

El 10% de los donantes de sangre sanos son positivos para los anticuerpos
anticardiolipina y el 1% lo son para el lúpico, pero después de un año, menos
del 1% continúan siendo positivos a las pruebas. Es excepcional identificar un
perfil de alto riesgo en una persona sana (García D, 2018).
¿Cómo se manifiesta?
El SAF se caracteriza clínicamente por fenómenos trombóticos multiorgánicos
y pérdidas fetales recurrentes (Cervera R, 2002; Miyakis S, 2006; Lim W, 2009;
Ruiz-Irastorza G, 2009; Kolitz T, 2019).

¿Cómo se diagnostica?
Para el diagnóstico de SAF se utilizan los criterios de Sapporo de 1999
modificados en el Congreso Internacional de Anticuerpos Antifosfolipídico de
Sidney en 2004 (Miyakis S, 2006; Cervera R, 2006; Ruiz-Irastorza G, 2009) que
considera el diagnóstico como definitivo si existe al menos un criterio clínico
y otro de laboratorio.

SAF catastrófico
Los órganos más afectados por orden de frecuencia son el riñón (73% de los
pacientes) (microangiopatía trombótica renal; la mayoría de los pacientes
tienen HTA grave o maligna, proteinuria moderada e insuficiencia renal
aguda que puede requerir diálisis hasta en un 25% de los casos), seguido de
los pulmones (60%) (SDRA), el sistema nervioso central (56%) (microinfartos
cerebrales, sufrimiento encefálico difuso, cuadros confusionales, convulsiones
o cefaleas), el corazón (50%) (microtrombosis miocárdicas, insuficiencia
cardíaca e infartos de miocardio) y la piel (47%) (livedo racemosa, necrosis
cutáneas, gangrena de las extremidades, púrpuras y hemorragias en astilla)
también puede afectar al sistema digestivo, hepático, suprarrenal, urogenital,
tiroideo y óseo.
El SAFC se define según los Criterios de Taormina del año 2002 (Asherson
RA, 2003):

¿Cómo se trata?
Se sugieren las recomendaciones terapéuticas del 13º, 14º y 15º Congresos
Internacionales sobre Anticuerpos Antifosfolípido (Erkan D, 2014; Bertolaccini
ML, 2014; Andrade D, 2017), de la Sociedad Española de Reumatología (SER)
(Cáliz Cáliz R, 2019) y de la Liga Europea contra el Reumatismo (EULAR)
(Tektonidou MG, 2019) (tabla 7).

Las mujeres con AAF o SAFO presentan un mal pronóstico sin medicación
preventiva durante la gestación, tienen un riesgo más elevado que el de la
población general de padecer trombosis, principalmente venosas, pérdidas
gestacionales y fracasos en las técnicas de reproducción asistida (Cáliz Cáliz R,
2019), pero desafortunadamente no hay evidencia sobre la efectividad de
ningún tratamiento.
Pacientes con SAFO (3 o más abortos antes de la 10ª semana o pérdidas fetales
de 10 o más semanas):
La dosis mínima óptima de AAS no está aclarada (Lockwood CJ, 2019). Sin
embargo, sí se ha observado que ajustar las dosis profilácticas de las
heparinas progresivamente por el peso de la paciente, a lo largo de todo el
embarazo, obtiene un mejor resultado que las dosis fijas (Ruffatti A, 2011).
Pacientes con SAFO (parto antes de la 34ª semana debido a preeclampsia grave
o eclampsia o insuficiencia placentaria):
Pacientes con morbilidad relacionada con los AAF (sin cumplir criterios
diagnósticos):
Existe aún menos consenso si cabe; valorar AAS y/o HBPM profilácticas o
intermedias en función del perfil de riesgo de la paciente (perfil de AAF,
enfermedad autoinmune concomitante, complicaciones durante embarazos
previos, nacimientos vivos previos, factores de riesgo suplementarios, etc.).

4. Otros tratamientos
Está por definir el papel de otras terapias como los péptidos dirigidos frente a
los dominios I o V de la β2GPI, la coenzima Q10 o los antagonistas de los
receptores de adenosina-difosfato (ticlopidina, clopidogrel, prasugrel, ticagrel
or).
