El PVM secundario, está asociado a otros trastornos del tejido conectivo,
como el síndrome de Marfan, síndrome de Ehlers-Danlos, osteogénesis CIE9424.0
imperfecta y pseudoxantoma elástico (Glesby MJ, 1989).
¿Cuál es su clínica?
La mayoría de los pacientes se encuentran asintomáticos durante muchos
años.
Son raros los casos de muerte súbita (<2%/año) por arritmias ventriculares
malignas, siendo más frecuente la fibrilación auricular.
¿Cómo se diagnostica?
El diagnóstico de prolapso de la válvula mitral (PVM) se basa
fundamentalmente en los hallazgos ecocardiográficos.
Los pacientes con PVM, pueden presentar anomalías esqueléticas como una
disminución del diámetro anteroposterior del tórax, escoliosis, pérdida de la
cifosis de la columna torácica y deformidad pectus excavatum.
Las indicaciones de cirugía son las mismas que para los casos de IM severa.
En ocasiones, cuando el prolapso se debe a la valva posterior puede repararse
con grandes posibilidades de éxito siendo una cirugía sencilla sin grandes
complicaciones. En el caso de la valva anterior, la reparación es más difícil con
un alto porcentaje de recidiva (Bonow RO, 2008).
