b) Sintomática
Dolor biliar: dolor intenso en epigastrio (infrecuente en
hipocondrio derecho), a menudo se irradia al hombro derecho,
forzando al paciente a permanecer en reposo, no alivia con
evacuaciones, habitualmente el dolor es constante y no cólico.
En 80% de pacientes, náusea y vómito. Con frecuencia es
episódico.
c) Complicaciones
Fiebre, ictericia, etc.
Diagnóstico de litiasis vesicular

visualización en ultrasonido de la litiasis vesicular

(arrow). The striated appearance strongly suggests the presence of intramural edema due to acute
cholecystitis.
(b) Contrast material-enhanced CT scan in the same patient demonstrates low-attenuation edema in the
thickened gallbladder wall (arrow). Despite the presence of severe acute cholecystitis, the image shows
almost no stranding or inflammatory reaction in the pericholecystic fat.
Diagnóstico de colecistitis aguda
mediante ultrasonido y TC
US: presencia de litos asociado a engrosamiento de la pared con halo hipogénico (signo de la doble
pared).
TC: engrosamiento mural y reforzamiento inflamatorio del parénquima hepático adyacente
Colecistitis gangrenosa
US image shows a linear membrane (white arrow) that has sloughed into the
gallbladder lumen. This membrane was seen to freely change position with changes
in the patient's position. Sludge and debris have dissected behind the necrotic wall
(black arrow).
Colecistitis enfisematosa
(a) US scan demonstrates collections of acrogenic bubbles (arrows) in the GB wall. The bubbles did not
seek gravitational dependency.
(b) (b) Plain abdominal radiograph of the right upper quadrant helps confirm the finding of
emphysematous cholecystitis. The complication of acute cholecystitis is most common in diabetic
patients.
Diagnóstico de colecistitis aguda
mediante TC
(a) CT scan shows a markedly edematous gallbladder wall (arrows) caused by acute cholecystitis. This
appearance should not be confused with that of pericholecystic fluid, which has many causes
besides acute cholecystitis. Note the spiculation of the lateral aspect of the hepatic flexure of the
colon from adjacent inflammation.
(b) CT scan caudad to a shows inflammatory stranding (arrow) in the pericholecystic fat. Although not
always present in acute cholecystitis, stranding may be a helpful sign. Gallstones are also seen.

Las imágenes iniciales muestran radioactividad en el hígado, corazón y grandes vasos, luego se
acumulará progresivamente en el parénquima hepático. La radioactividad se irá depurando del
parénquima hepático hacia los conductos biliares derecho e izquierdo, hepático común, colédoco y
vesícula, debiendo ésta aparecer a los 20-40 minutos de comenzado el examen. Debe observarse el
paso del radiotrazador hacia la cavidad intestinal. La cantidad de radiotrazador y el tiempo de ingreso
a
la vesícula dependen de factores como el estado nutricional del paciente, medicamentos que ingiere y
el tono del esfínter de Oddi. Ocasionalmente puede verse una pequeña cantidad del radiotrazador
que
refluye hacia la cavidad gástrica, que debe interpretarse como normal.
