• Las enfermedades degenerativas de las articulaciones resultan de un desequilibrio
entre los procesos anabólicos y catabólicos controlados predominantemente por
los condrocitos
• Degradación progresiva de los componentes de la matriz extracelular del cartílago
articular con posibles componentes inflamatorios secundarios.
• Todavía no se ha identificado un factor de iniciación único. El insulto primario
puede ser local o sistémico y puede ser mecánico, químico o inflamatorio.
• El resultado es la ruptura de los condrocitos y/o las células sinoviales, liberación
de enzimas proteolíticas y degradación de la matriz.
• Puede estar localizada focalmente en una articulación y ser clínicamente
silenciosa.

The therapy is focussed on the relevant items regarding the physical examination process liTke
posture, neuromotorcontrol and stress management, muscle and joint system in the orofacial,
cervico-thoracic spine and shouldergirdle area
(Townsen et al, 2001; Michelotti et al, 2004, 2005; Mc Neely et al 2006; Medlicott et al, 2006).
Fármacoterapia
• NSAIDs are rst-line agents typically used for 10 to 14 days for initial treatment of
acute pain. Only naproxen (Naprosyn) has proven benefit in reduction of pain.
• Habit reversal techniques using biofeedback proved efficient in lowering the general tension
of the patient (Crider & Glaros, 1999). In patients with chronic pain, the management
strategy must take into account the coping styles and locus of control.
Biofeedback is a behavioral training program that teaches the control of certain autonomic
reactions. Its goal is to reduce or eliminate pain through learned control of physiological
responses of the body.
Manipulación craneosacral
• Craniosacral therapy is the application of light pressure to the head allegedly completed to release
restrictions in the craniosacral system. It is used for a variety of conditions especially for TMJ disorder. It is
done by some osteopaths, massage therapist, chiropractors, dentists and physical therapists. The other
terms used for this form of treatment are cranial osteopathy, cranial therapy, bio-cranial therapy,
craniopathy and sacro-occiptal technique (SOT).
• Clinical evidence does not support the use of craniosacral therapy as randomized, blinded and placebo
controlled outcome studies have not been published to establish its efficacy. According to the British
Columbia Office of Health Technology Assessment and other authors, the theory is invalid and
practitioners cannot reliably measure the claimed outcomes. There is no evidence to substantiate that the
bones of the head can be manipulated and that this manipulation will treat, alter, or cure a disorder of any
nature (Hartman, 2002; Kazanjian, 1999).

• Distraction osteogenesis may make its own
contribution to TMJ OA in the future (van
Strijen 2001).
Cirugía ortognática
• After relief of pain, patients should be informed that orthognathic surgery may be indicated
in cases of pronounced forms of malocclusion. This information should include the effects,
benefits and risks.
• There are studies indicating that orthognatic surgery results in better chewing ability (van den
Braber et al, 2005 ) and does not lead to an increase in TMD symptoms (De Boever et al,
1994; Egermark et al, 2000, Farella et. al. 2006).
• However, in case of pre-existing TMJ dysfunction, orthognathic surgery may also lead to
worsening of the symptoms (Wolford et al, 2003). Management of pain and dysfunction
before surgery is mandatory.
• After surgery, there is a risk for a progressive condylar resorption in women with a
mandibular angle > 37 degrees and pre-existing dorsally migrated condyle. (Hoppenreijs et al,
1998).
