AINES:
• Evidencia moderada en cólico renal y dismenorrea.
• RAM: Ulceración- Exacerbación de EII-enteropatía.
• Usar en combinación a IBP o usar Cox-2.
PARACETAMOL:
• Preferido en dolor visceral crónico leve-
moderado.
• Continuado al escalar a analgésicos potentes.
• Sin ECCR que lo avalen en dolor visceral, pero amplio
uso.

Analgésicos potentes:
OPIOIDES:
• Larga duración usados en dolor visceral orgánico
crónico.
• Incierto beneficio en trastornos funcionales crónicos.
• Morfina, Tramadol y Oxicodona útiles en pancreatitis
crónica.
• Morfina aumenta umbrales mecánicos esofágicos.
• Oxicodona aumenta umbrales mecánicos y térmicos.
• Tramadol aumenta umbral de distensión rectal, con
menos RAM que morfina.
• En animales: Tramadol disminuye hiperalgesia viscero-
visceral en endometriosis y urolitiasis.

• Uso concomitante de Naloxegol (Antagonista
periférico selectivo. Movantik {Astra}12.5-25mg comp.)
en constipación inducida por opioides en dolor crónico
no oncológico (No en Chile).
• Uso concomitante de laxantes.
Síndrome de Intestino Narcótico:
• 5 % de pacientes en TTO con opioides.
• Aumento paradójico del dolor al aumentar dosis.
• Equivalente a la hiperalgesia inducida por opioides en
dolor somático.

• ECCR: Pregabalina mejor que placebo a 3 sem. en
