atendido mi consultorio privado ubicado en Av. Maclovio Herrera 5-7, Bolívar,
84060 Nogales, Son. Con asistencia regular 3 veces por semana los días:
Lunes, miércoles y viernes. Desde el 20 de Junio de 2020 a la fecha.
Con Diagnostico: Depresión severa debido a secuelas un accidente
automovilístico ocurrido en Junio de 1991
Recomendaciones: Continuar con evaluación Psicológica y asistir algún
grupo de autoayuda regularmente.

Hago de su conocimiento que el examinado ha mostrado: una actitud

positiva durante su tratamiento y está dispuesto a seguir las
recomendaciones para lograr su total recuperación

Se expide la presente Constancia a solicitud del interesado (a) para los fines
que estime conveniente.

Nogales, Son. With regular attendance 3 times a week on: Monday,
Wednesday and Friday from June 20, 2020 to date.
With Diagnosis: Severe depression due to the aftermath of a car accident that
occurred in June 1991
Recommendations: Continue with psychological evaluation and attend a self-
help group regularly.

I inform you that the examinee has shown: a positive attitude during his
treatment and is willing to follow the recommendations to achieve his full
recovery

This Certificate is issued at the request of the interested party for the purposes
it deems appropriate.
