Con una prevalencia aproximada de 1 entre cada 2.000-5.000 personas. Es la alteración hereditaria más frecuente
de los eritrocitos asociada a anemia hemolítica hereditaria en personas procedentes del norte de Europa.

FISIOPATOLOGIA
• Alteraciones de la anquirina y de la espectrina.
• La pérdida del área de superficie de la membrana sin pérdida
proporcional del volumen celular hace que los eritrocitos tengan
menor capacidad de deformación, lo que dificulta su paso
desde los cordones esplénicos hacia los senos esplénicos, con
el consiguiente atrapamiento y destrucción prematura de los
eritrocitos con EH por el bazo.

En el período neonatal, la EH es una causa significativa de hemólisis y puede
manifestarse como anemia e hiperbilirrubinemia de gravedad suficiente como
para precisar fototerapia, transfusiones o exanguinotransfusión.

Afecta en el mundo a unas 400 millones de personas. Su prevalencia es mayor en zonas

La G6PD es una enzima que interviene en la vía de las pentosas-fosfato y
que proporciona a las células capacidad reductora, esencial en muchas
reacciones enzimáticas para evitar el estrés oxidativo celular.

FISIOPATOLOG

globina.

FISIOPATOLOGIA
Se forman tetrámeros de globina α y
aparecen como inclusiones dentro de
los ERI. Las cadenas de globina α
libre y las inclusiones son muy
inestables, precipitan dentro de los
precursores de los ERI, causan
lesiones en la membrana eritrocitaria y
acortan la vida de los ERI, provocando
anemia y aumento de la producción
eritroide
En los síndromes de talasemia α se
produce un exceso de cadenas de globina
β y γ y aparición de la hemoglobina de
Bart (γ4) durante la vida fetal y de HbH
(β4) tras el nacimiento.
Un feto con la forma más grave de
talasemia α (hidropesía fetal) presenta
anemia intrauterina

HEMOGLOBINA

α-talasemia menor o rasgo α-talasémico:
ausencia de dos de los cuatro genes de las cadenas
α.
• Hemoglobinopatía H: ausencia de tres de los
cuatro genes de las cadenas α. Se forman
tetrámeros de cadena beta que precipitan en el
hematíe en forma de cuerpos de inclusión de
hemoglobina H (β4) dando al hematíe una imagen
multipunteada similar a “pelotas de golf”. Asocian
hemólisis crónica y esplenomegalia.

• Hemoglobina de Bart (hidropesía fetal): es

incompatible con la vida (muerte intraútero o poco
después del nacimiento). Existe ausencia absoluta
de cadenas α, con formación de tetrámeros de
cadena gamma (γ4).

La ictericia fisiológica es una situación muy frecuente (60% de recién nacidos) en el neonato a término, y
se caracteriza por ser monosintomática, fugaz (2º a 7º día), leve (bilirrubinemia inferior a 12,9 mg/dL si
recibe lactancia artificial o a 15 mg/dL si recibe lactancia materna), y de predominio indirecto.

ICTERICIA PATOLOGICA
Una ictericia será patológica (6% de recién nacidos) cuando se inicie en las primeras 24 horas, se acompañe de
otros síntomas, la bilirrubina aumente más de 5 mg/dL diarios, sobrepase los límites definidos para ictericia
fisiológica, la fracción directa sea superior a 2 mg/dL o dure más de una semana en el RN a término.

Con independencia de la etiología, el tratamiento está encaminado a prevenir
la neurotoxicidad relacionada con la bilirrubina indirecta sin ocasionar daños
indebidos. La fototerapia o, si esta fracasara, la exanguinotransfusión siguen
siendo las modalidades terapéuticas fundamentales para mantener la
bilirrubina total máxima por debajo de los valores patológicos.
