Ciertos cuestionarios han sido validados para ayudar a objetivar el control del
asma. Entre ellos se encuentran el Asthma Control Test (ACT) para mayores de
12 años2, el children ACT para niños de 4 a 11 y el Composite Asthma Severity
Index (CASI)3.

En función del grado de control que se logra con el tratamiento, el paciente debe
ser ubicado en alguno de los 5 niveles (ver capítulo de manejo del asma). En los
niveles más altos, el 4 y el 5, el tratamiento controlador debería incluir un
corticoide inhalado a alta dosis combinado con un broncodilatador de acción
prolongada (y eventualmente un antileucotrieno), y en los casos de control
insuficiente se considerará el agregado de corticoides sistémicos y/u Omalizumab.
Como vemos, estas preguntas pueden tener formas a veces fáciles u otras
difíciles de detectar y es tarea del médico identificarlas para intentar controlar la
enfermedad con los niveles de tratamiento usuales, antes de considerar que el
paciente es realmente un asmático verdaderamente grave.
Según el mecanismo fisiopatogenico, los endotipos pueden ser Th2 alto y Th2
bajo Para identificar los endotipos se utilizan los biomarcadores. Se consideran
biomarcadores Th2 a: eosinofilia (+300/mm3), FeNO y eosinófilos en esputo. Los
marcadores Th2 bajo son: neutrófilos en esputo, IL-17 e IL-6. Muchos otros
marcadores biológicos se encuentran en estudio para poder identificar el
mecanismo fisiopatológico interviniente.
TRATAMIENTO FARMACOLOGICO
Los niños con asma severa son tratados con corticoides inhalados a dosis medias
o altas combinados con beta agonistas de acción prolongada (como se describe
en el capítulo de tratamiento del asma) y otra terapia adicional (Figura 3).
Los pacientes que tienen eosinofilia, FeNO elevado y periostina elevada suelen
tener mejor respuesta al omalizumab28. Diversos estudios demostraron que
reduce los días de síntomas (especialmente en otoño y en primavera)29,
aumenta la función pulmonar30 y los scores de calidad de vida31.
El costo de la terapia con omalizumab sigue siendo muy elevado, los efectos
adversos a largo plazo no han sido evaluados en niños y la duración óptima del
tratamiento en pediatría aún no se ha determinado.
MEDIDAS ADICIONALES
Si en el asma en general son importantes, en el asma severa lo son más las
medidas complementarias a la terapéutica. Entre ellas mencionamos a:
Educación del paciente y su familia
Adecuada relación médico-paciente, que incluye la posibilidad de
comunicación directa en las exacerbaciones y plan de acción escrito para el
tratamiento de mantenimiento y las crisis obstructivas
Reforzar la importancia de la adherencia a los tratamientos
Instrucción y revisión periódica de la técnica de administración inhalatoria
Medidas de control ambiental, incluyendo evitar el tabaquismo pasivo y
activo
Evitar la exposición a los alérgenos desencadenantes conocidos
Alimentación y actividad física adecuadas. Reducción del peso si el
paciente es obeso.
Tratamiento psicoterápico apropiado en casos de stress, ansiedad o
depresión.
Vacunación antigripal todos los años.
Controlar la posible aparición de efectos adversos de las medicaciones
utilizadas.
