Las quinolonas orales, principalmente ciprofloxacino (500 mg/2 veces al día) y norfloxacino
(400 mg/2 veces al día), son muy eficaces incluso en pacientes con litiasis biliar, situación que
ocasiona numerosos fallos del tratamiento con otros antibióticos. Esta superior eficacia se debe
a que presentan una mayor penetración intracelular, y sólo precisan ser administradas durante
4 semanas. Además, no parecen inducir cambios en la flora intestinal, pudiendo
incluso erradicar el reservorio intestinal. Sin embargo, aunque se ha afirmado que no
favorece la aparición de cepas resistentes, se ha detectado algún caso de Salmonella
resistente al ciprofloxacino, lo cual hace dudar sobre la conveniencia de utilizar las
quinolonas –como tratamiento de primera elección- para esta situación clínica, al ser
antibióticos muy eficaces para el tratamiento de otras infecciones.

around them. The virus can survive on these objects for many hours. Rhinoviruses produce
clinical illness in only half of those infected, and thus many asymptomatic individuals are
capable of spreading the virus. Rhinovirus "colds" affect people in temperate climates most
frequently in the early fall, which may reflect the return to school rather than any change in the
virus itself. Rates of infection are highest in infants and children, who are also the primary vector
introducing colds into family units. Secondary infections occur in ~50% of family
members, especially other children. Although many different rhinovirus serotypes may be found
in a given community, only a few predominate during a specific cold season. They tend to be
the newly categorized serotypes, suggesting that a gradual antigenic drift occurs.

Uric acid is formed by the breakdown of purines and by direct synthesis from 5-phosphoribosyl
pyrophosphate and glutamine. In humans, uric acid is excreted in the urine. The normal blood
uric acid level in humans is approximately 4 mg/dl. In the kidney, uric acid is filtered,
reabsorberd, and secreted. Normaly, 98% of the filtered uric acid is reabsorbed and
the remaining 2% makes up approximately 20%of the amount excreted. The remaining 80%
comes from the tubular secretion. The uric acid excretion on the purine-free diet is about 0.5 g/24
h and on a regular diet about 1 g/24 h.

Erectil disfunction (ED) is essentially a vascular disease. It is often associated with other
vascular diseases and conditions such as diabetes, hypertension, and coronary artery disease.
Other conditions associated with ED include neurologic disorders, endocrinopathies,
benign prostatic hyperplasia, and depression. Conditions associated with reduced
nerve and endothelium function, such as aging, hypertension, smoking,
hypercholesterolemia, and diabetes, alter the balance between contraction and relaxation
factors. These conditions cause circulatory and structural changes in penile tissues,
resulting in arterial insufficiency and defective smooth muscle relaxation. In some patients,
sexual dysfunction may be the presenting symptom of these disorders. Additionally, ED is
often an adverse effect of many commonly prescribed medications. Some psychotropic drugs
and antihypertensive agents are associated with ED. Trauma that affects the neurologic or
vascular components can also lead to ED. Men with severe Peyronie disease, an inflammatory
vasculitis, may have enough scar tissue in the corpora to impede blood flow. Men with sleep
disorders commonly experience ED. Another important consideration is the hormonal status
of the patient. Hypogonadism that results in low testosterone levels adversely affects libido and
erectile function. Hypothyroidism is a very rare cause of ED. Most patients with ED have
multiple etiological factors; thus, assessing how much each is contributing to the problem is
difficult. Because most men with ED have an organic cause, a thorough evaluation is
necessary to correctly identify the specific etiology in any given individual.

La uncinariasis es una infestación por nematodos que puede ser causada por cualquiera de los
dos parásitos peligrosos para el ser humano, Ancylostoma duodenale o Necator americanus.
Ambas formas de la enfermedad se caracterizan por dolor abdominal y anemia ferropénica. Los
gusanos penetran en el cuerpo como larvas atravesando la piel(usualmente penetran por los
pies, por lo que la opción C es la correcta), se desplazan a través del aparato circulatorio hasta
los pulmones y ascienden por el tracto respiratorio, donde son deglutidos. En el tracto

are allergic to penicillin, and some cephalosporins (ie, cefotaxime, ceftriaxone, cefuroxime)
used to treat pediatric patients. Meningococcal resistance to penicillins has occurred; the
mechanism of resistance involves altered penicillin-binding proteins. Sulfonamides have
a limited role in meningococcal infections because of the resistance of serogroups A, B, and
C. Duration of treatment is dictated by clinical response and manifestation of disease, although
10-
14 days should be sufficient with a sensitive organism. Individuals with at least 4 hours of close
contact during the week before onset of illness have an increased risk of being
infected. Individuals at risk include housemates, daycare contacts, cellmates, or individuals
exposed to infected nasopharyngeal secretions (eg, through kissing, mouth-to-mouth
resuscitation, intubation, suctioning). Rifampin and ciprofloxacin are commonly used for
chemoprophylaxis. Other agents include ceftriaxone and azithromycin. A single dose of
intramuscular ceftriaxone may be used in children or adults. Spiramycin is the primary
prophylactic regimen used in many European countries. Vaccination with monovalent A;
monovalent C; bivalent A-C; or quadrivalent A, C, Y, and W-135 vaccine should be an
adjunct to antibiotic chemoprophylaxis of susceptible contacts in epidemics. The eradication of
carriage is also indicated in the index case unless third-generation cephalosporins have been
used. A single intramuscular dose of an oily suspension of chloramphenicol has been shown
to be as effective as 5 days of penicillin in persons with meningococcal meningitis, and this
may be useful in resource-poor countries.
