MÉTODOS COMPLEMENTARIOS
• Rx panorámica de pelvis y axial de cadera (3-6 meses del inicio síntomas)
• RMN pelvis (extensión de la necrosis en los estadios iniciales)
• Con o sin gadolinio índice perfusión (etapa condensación)
• Índice deformidad (etapa reosificación)

• 1° condensación (isquemia) – epífisis esclerótica (situación de
isquemia)
• 2° fragmentación (fisuras longitudinales en la epífisis que
puede afectar metáfisis como quistes) superpuestas,
duración hasta 2
• 3° reosificacion (regresa el flujo sanguíneo, deposito calcio años
nuevo, nuevo hueso)
• 4° curación: remodelación (secuelas)

• Rx panorámica pelvis y axial en la etapa curación
• Clase I: normal
• Clase II: cabeza femoral esférica
• Coxa magna
• Cuello femoral corto
• Acetábulo anormalmente empinado
• Clase III: cabeza femoral no esférica pero tampoco plana con alguna de las características de la
clase II
• Clase IV: cabeza femoral plana pero congruente debido a la remodelación acetabular con las
anormalidades de la clase II
• Clase V: cabeza femoral plana y no congruente debido a la ausencia de remodelación
acetabular, cuello femoral normal

TRATAMIENTO
Conservador:
• Modificación actividad (regreso progresivo al deporte)
• Tenotomía aductores (restituir abducción) y 6 semanas yeso en abducción luego:
• Férulas abducción: 18hs x 4 semanas luego 12hs hasta pasar la etapa fragmentación
• FKT (ejercicios diarios)
• Osteotomías:
• Fémur: varizante Mas utilizada a nivel mundial
• Pelvis: Salter / triple osteotomía Mas utilizada en Norteamérica,
Suramérica y Australia

TRATAMIENTO CX
• Osteotomia varizante femur:
• Indicación: 8-11 años – Estadio temprano enfermedad (1-2a) – Adecuada abducción (>30° ABD
pasiva en extensión)
• 6-8 años – Extrusión – Adecuada abducción
• Corrección de 10-20° varo
• Indicaciones: >10 años – estadio temprano (pre colapso o poco colapso) - >50%
hipoperfusión de la cabeza femoral. PO: sacar carga
• Repeterir RMN a los 3-4 meses
TRATAMIENTO CX
Artrodiastasis: (tutor externo 3-4 meses)
• Prevenir la deformidad y restituir la forma redonda de la cabeza
• Podría incrementar la revascularización y la velocidad de curación del hueso
• Podía incrementar la movilidad de la cadera estirando las partes blandas contracturadas de
alrededor de la cadera
• En comparación a bifosfonatos
• Acción especifica
• No inhibición crecimiento longitudinal hueso
