RESUMEN
ABSTRACT
Pain associated with orthodontic treatments is a common concern for both patients
and dental health professionals, effective pain management is essential to improve
the patient experience during orthodontic treatment. Analgesics have been widely
used to control pain in various medical and dental situations, including orthodontics.
The objective of this study was to carry out an exhaustive literature review on the use
of analgesics in pain control in orthodontics. The different types of analgesics
manipulated, their mechanisms of action, efficacy and safety in orthodontics were
analyzed. To carry out the bibliography review, databases such as Pubmed and
Google Scholar were used, with search limits being documents published between
the years 2019 and 2023. For that, 11 articles that met the search criteria were used.
It was concluded that pain management in orthodontic treatments shows the
importance of managing it without interfering in the inflammatory process. After
analyzing several non-steroidal anti-inflammatory drugs and their influence on
inflammation by altering the treatment, it is considered that Paracetamol is the most
appropriate and first choice due to its specific action on the central nervous system,
without altering the synthesis of prostaglandins and allowing tooth movement, leaving
COX2 and Opioid inhibitors as options

AINES
Los Aines son uno de los principales fármacos usados en el control del dolor,
ya que su función está dada en la inhibición de las vías de la ciclooxigenasa
llevando así un efecto antiinflamatorio ligado a la producción de prostaglandina
E2 que aumenta la permeabilidad vascular, activación de osteoclastos y
reabsorción ósea; pero en ortodoncia estos efectos favorecen al movimiento
dentario por lo que su uso debe ser limitado. (7)
El Tenoxicam perteneciente al grupo de los oxicames que tiene una vida media
de 67 horas en el cuerpo sumado a una toma diaria, junto al Ibuprofeno
indicado para el control del dolor tras procedimientos de ortodoncia acelerada o
corticotomía son una potente elección de medicamento analgésico, pero al
igual que el naproxeno su acción antiinflamatoria limitan su uso en ortodoncia.

Son un sustituto razonable de los AINE cuando el tratamiento del dolor dental
no es la mejor opción y se presentan sintomatologías severas. Se utilizan cada
vez más con fines médicos y no médicos, es fundamental recordar que estos
medicamentos tienen efectos secundarios y contraindicaciones que restringen
su uso como: depresión respiratoria, sedación, náuseas, vómitos,
estreñimiento, miosis, tolerancia, dependencia física con uso prolongado y
posible adicción. (14)
