Importancia de los Trastornos por Tics (TT)
Los TT pueden producir diverso grado de deterioro en la vida de los personas:
a) Impactar en el desempeño personal ( rutinas, cole, trabajo) y social del paciente
b) Producir diverso grado de impacto funcional, dolor y hasta daño tisular
c) Ocasionar baja autoestima, vergüenza y ser motivo de burla
• El grado de tolerancia hacia los tics es individual para cada paciente y familia

El espectro de los trastornos por tics
• Los TTics son considerados hoy trastornos del neurodesarrollo ( base biológica)
• Implican una disfuncion de los circuitos fronto-estriatales
Abarcan un espectro que puede incluir
• Trastornos por Tics Transitorios ( mas de 4 semanas pero menos de 1 ano) a

Criterios DSM-5
• Trastorno por Tics Provisional: tic motores y/o fonicos unicos o multiples ,
al menos 2 semanas pero menos de 1 ano desde el inicio , y el pte nunca
reunio criterios para ST o TT persistente
• Trastorno por Tics Persistente: solo tics motores o fonicos , durante mas de
1 ano, de inicio antes de los 18 anos
• Sindrome de Tourette : 2 o mas tics motores y al menos 1 tic vocal , no
necesariamente concurrentes , tics presentes al menos durante 1 ano ,
inicio antes de los 18 anos
• En todos los casos los tics NO se deben a efectos fisiologicos directos de
sustancias ( por ej, cocaina) ni a otra condicion medica general ( por ej

Qué son los Tics
• Son movimientos repentinos, rapidos , breves, recurrentes, involuntarios
• Reproducen aspectos de la conducta humana normal, pero se dan fuera de
contexto por una sensibilidad selectiva a claves ambientales internas y externas
• Pueden ser motores (movimientos) o fónicos ( sonidos o ruidos provocados por el
pasaje de aire por la garganta o nariz)
• Pueden ser simples ( un solo grupo muscular) o complejos (secuencias de
patrones complejos de movimientos)

Abstract
Youth with chronic tic disorders (CTD) experience social problems that have been associated with functional impairment and a
diminished quality of life. Previous examinations have attributed social difficulties to either tic severity or the symptom severity of coexisting
conditions, but have not directly explored performance deficits in social functioning. This report examined the presence and characteristics of
social deficits in youth with CTD and explored the relationship between social deficits, social problems, and quality of life. Ninety-nine youth
(8–17 years) and their parents completed a battery of assessments to determine diagnoses, tic severity, severity of coexisting conditions,
social responsiveness, and quality of life. Parents reported that youth with CTD had increased social deficits, with 19% reported to have
severe social deficits. The magnitude of social deficits was more strongly associated with inattention, hyperactivity, and oppositionality than
with tic severity. Social deficits predicted internalizing and social problems, and quality of life above and beyond tic severity. Social deficits
partially mediated the relationship between tic severity and social problems, as well as tic severity and quality of life. Findings suggest that
youth with CTD have social deficits, which are greater in the presence of attention-deficit hyperactivity disorder and obsessive compulsive

CBIT no es una cura para el ST, sino una herramienta que ayuda a las personas a controlar mejor
sus tics y mejora su calidad de vida.
Los pacientes aprenden un conjunto de habilidades que se pueden aplicar por el resto de sus

• Tourette association of America desde el 2001 comenzo a desarrollar
TRH , terapia altamente estructurada para tratar los tics ( la menos 9
ses en 10 semanas en manos de rerapeuta experto )
• Hay que derribar mitos comunes como que los tics son voluntarios y
facilmente controlables , o que son una mana o que los chicos los
hacen para llamar la atencion
• No siempre van a desaparecer , la terapia CIBT no es una cura ,sino
mas bien una herramienta que ayuda mucho a manejar los tics y a
reducir las influencias negativas de estos en la vida
• No todos son candidatos a CBIT , la comorbilidad debe tratarse 1ro

Tratamiento farmacologico de los tics
• Se considera un tto farmacologico de los tics cuando los ptes requieren un tto
activo y la TC no esta disponible, o no es factible por falta de adherencia al
protocolo , o su eficacia fue limitada
• Considerar siempre la historia natural de los TT, y el curso fluctuante
• Periodicamente reducir la medicacion para ver si aun es necesaria
• No se sabe aun que efectos tendrán los farmacos para tics en el cerebro en
desarrollo, y en el curso natural del ST , y son solo sintomaticas , no curativas

Los tics se medican cuando…
1. causan gran disconfort , dolor o injuria ( por su alta frecuencia o por AI)
fracturas, lesion de discos o articulaciones , oculares , golpes , latigazo, PU
2. causan problemas sociales sostenidos: tics motores complejos persistentes o
fonicos muy fuertes que causan aislamiento social, bullying o estigma social, y
ya se intervino psicoeducando la clase y el entorno y con psicoterapia
3. causan pb emocionales severos : vergüenza ,sintomas depresivos reactivos
4. Causan interferencia funcional en la tarea , impiden el sueno, el habla y la
comunicación , lo academico ( rolido de ojos), el trabajo o la recreacion
