Para dar cumplimiento a lo establecido en la ley 1581 de 2012 ley de protección de datos personales y el decreto reglamentario 1377 de
2013, le informamos que los datos personales que han sido suministrados a través de los diferentes medios a nuestra compañía Alimentos
SAS S.A.S. con NIT 800.195.623-7, son recolectados y conservados en nuestra base de datos. Teniendo como propósito salvaguardar la
información personal comprometiéndonos con la política de confidencialidad de acuerdo a lo establecido por la le, realizando nuestro mayor
esfuerzo para brindar un servicio óptimo.

In order to comply with provisions of Colombian law 1581 of 2012 and Colombian regulatory decree 1377 of 2013 which refer to the protection
of personal data, we would like to inform you that personal data provided through different means to our company Alimentos SAS S.A.S. with
Colombian tax ID NIT 800.195.623-7, are collected and preserved in our database and safeguarded by committing ourselves to the policy of
confidentiality in accordance with what is established by law, making our greatest effort to provide an optimal service.

1. Autorizo a Alimentos SAS S.A.S. para que en el desarrollo de su objeto social y sus actividades comerciales recolecte, utilice,
almacene, custodie, suprima, transfiera, transmita y en general procese mis datos personales.
I authorize Alimentos SAS S.A.S. to collect, use, store, transfer, transmit, and generally process my personal data in the
development of its corporate purpose and its business activities.
2. Conozco y acepto expresamente el tratamiento de mis datos personales por parte de ALIMENTOS SAS S.A.S. los cuales son
utilizados exclusivamente en ejercicio de las actividades comerciales propias de ALIMENTOS SAS S.A.S

its commercial activities.
3. Conozco que mis derechos como titular de la información son los previsto por la ley, como el de conocer, actualizar, rectificar y
suprimir mi información personal, así como el de revocar el consentimiento otorgado y presentar quejas ante la SIC por infracciones
a los dispuestos en la ley.
I know that my rights as the holder of the information are those provided by law, such as knowing, updating, rectifying and deleting

5. LA CONTRAPARTE declara bajo la gravedad de juramento dando certeza de que lo aquí consignado es información veraz y
verificable para el Sistema de Autocontrol del Riesgo Integral de Lavado de Activos y Financiación del Terrorismo, lo siguiente:
THE COUNTERPART declares under oath giving certainty that what is stated here is true and verifiable information for the Self-
Control System for the Comprehensive Risk of Money Laundering and Terrorist Financing, the following:

 Que los recursos, fondos, dineros, activos o bienes relacionados en el presente formato y utilizados para la ejecución de la
relación comercial, no provienen de ninguna actividad ilícita de las contempladas en el Código Penal Colombiano o en
cualquier otra norma que lo modifique o adicione, ni serán utilizados para efectos de financiar actividades terroristas.
That the resources, funds, money, assets or goods related in this format and used for the execution of the commercial

relationship, do not come from any illicit activity of those contemplated in the Colombian Penal Code or in any other regulation
that modifies or add, nor will they be used for the purpose of financing terrorist activities.

 Que LA CONTRAPARTE o sus socios o administradores, no tienen registros negativos en listas de control de riesgo de lavado
de activos o financiación al terrorismo nacionales o internacionales que defina ALIMENTOS S.A.S, y que puedan ser
consultadas por esta.
That THE COUNTERPART or its partners or administrators do not have negative records in national or international asset
laundering or terrorist financing risk control lists defined by ALIMENTOS S.A.S, and that may be consulted by it.

 Que no incurren sus actividades en ninguna actividad ilícita de las contempladas en el Código Penal Colombiano o en
cualquier otra norma que lo modifique o adicione.
That their activities do not incur in any illicit activity contemplated in the Colombian Penal Code or in any other regulation that
modifies or adds to it.
 LA CONTRAPARTE acuerdan no permitir a sus socios o accionistas, representantes, empleados o contratistas el ofrecimiento
de incentivos, atenciones, cortesías u obsequios a empleados y/o funcionarios de la otra Parte, a fin de obtener con ello

beneficios propios o para sus socios o accionistas, representantes o contratistas, y aceptan que toda falta propia o de sus
funcionarios en torno a la transparencia con motivo de la negociación, celebración o ejecución del presente Contrato
constituye un incumplimiento grave. Será obligación de cada una de LAS PARTES poner en conocimiento de la otra cualquier
indicio o evidencia que vincule o pueda vincular a sus empleados o contratistas en las conductas aquí descritas.
THE COUNTERPART agree not to allow their partners or shareholders, representatives, employees or contractors to offer
incentives, hospitality, courtesies or gifts to employees and/or officials of the other Party, in order to obtain benefits for
themselves or their partners. or shareholders, representatives or contractors, and accept that any failure of their own or of their
officials regarding transparency due to the negotiation, execution or execution of this Agreement constitutes a serious breach. It
will be the obligation of each of THE PARTIES to inform the other of any indication or evidence that links or may link their
employees or contractors in the conduct described herein.

 Que LA CONTRAPARTE se compromete a realizar todas sus actividades con ética empresarial y el código de buen gobierno
corporativo. Para el efecto, LA CONTRAPARTE se compromete a cumplir los requisitos que el Sistema de Autocontrol y
Gestión del Riesgo de Lavado de Activos y Financiación del Terrorismo – SAGRILAFT le impone, dentro de los que se
encuentran, entregar información veraz y verificable y a actualizar su información institucional, comercial y financiera por lo
menos una vez al año, o cada vez que así lo solicite ALIMENTOS S.A.S, suministrando la totalidad de los soportes
documentales exigidos. El incumplimiento de esta obligación faculta a ALIMENTOS S.A.S para terminar de manera inmediata
y unilateral cualquier tipo de relación que tenga con LA CONTRAPARTE. LA CONTRAPARTE autoriza a ALIMENTOS S.A.S
realizar consultas a través de cualquier medio, por sí mismo o a través de un proveedor para efectuar las verificaciones
necesarias para corroborar la información aquí consignada.
That THE COUNTERPART undertakes to carry out all its activities with business ethics and the code of good corporate
governance. For this purpose, THE COUNTERPARTY undertakes to comply with the requirements that the Self-Control and
Risk Management System for Money Laundering and Financing of Terrorism - SAGRILAFT imposes on it, among which are, to

deliver truthful and verifiable information and to update its information institutional, commercial and financial at least once a

year, or whenever requested by ALIMENTOS S.A.S, supplying all the required documentary supports. Failure to comply with
this obligation empowers ALIMENTOS S.A.S to immediately and unilaterally terminate any type of relationship it has with THE

COUNTERPARTY. THE COUNTERPART authorizes ALIMENTOS S.A.S to make inquiries through any means, by itself or
through a supplier to carry out the necessary verifications to corroborate the information contained herein.

De acuerdo a lo anterior autorizo de manera voluntaria y explicita a ALIMENTOS SAS S.A.S para tratar mis datos personales de acuerdo con
la política de tratamiento de datos personales de la compañía.

In accordance with the above, I voluntarily and explicitly authorize ALIMENTOS SAS S.A.S to treat my personal data in accordance with the
company's personal data treatment policy.
