3107: Infección por VIH y sida: evolución y pronóstico

INTRODUCCIÓN
Con la mejora en el tratamiento, la esperanza de vida de los pacientes con apego terapéutico es casi normal. Un estudio poblacional realizado en
Dinamarca encontró que las personas infectadas con VIH a los 25 años de edad sin hepatitis C tenían una esperanza de vida similar a los individuos de
25 años no infectados. Por desgracia, no todas las personas infectadas con VIH tienen acceso al tratamiento. Los estudios muestran de manera
consistente un menor acceso al tratamiento para individuos de raza negra, indigentes y consumidores de drogas inyectadas. A los pacientes cuya
enfermedad avanza aun con el esquema apropiado, debe proveerse un cuidado paliativo meticuloso (véase el cap. 5), con atención en el control del
dolor, las necesidades espirituales y la dinámica familiar (familia biológica y elegida).
Cuándo referir
Se deben remitir con un especialista los pacientes con infección por el VIH en quienes no es posible suprimir por completo la carga viral con
alguno de los regímenes iniciales recomendados.
La consulta con un especialista es en especial importante para los pacientes con una carga viral detectable y tratamiento antirretroviral; aquellos
que no toleran los fármacos tradicionales, aquellos que necesitan quimioterapia por vía general; y aquellos con infecciones oportunistas
complicadas, sobre todo cuando se necesita algún procedimiento invasivo o experimental.
Cuándo hospitalizar
Pacientes con infecciones oportunistas y un problema agudo (p. ej., con fiebre, con cambios mentales rápidos o con insuficiencia respiratoria) o que
necesitan administración intravenosa de fármacos.
