Título Científico
Ensayo clínico 3 que evalúa la isquemia perioperatoria
Justificación
El sangrado en una cirugía no cardiaca está fuertemente asociado con un aumento de las complicaciones
cardiovasculares y de la mortalidad a los 30 días después de la cirugía. El acido tranexámico (ATX) podría prevenir
el sangrado postoperatorio. Se ha comprobado que su administración intravenosa disminuye el sangrado y la
transfusión en cirugía ortopédica; sin embargo, existen pocos estudios en pacientes sometidos a otro tipo de cirugía
no cardiaca. Además, todavía no se ha establecido su seguridad en el contexto de la cirugía no cardiaca (es decir,
su efecto sobre la trombosis arterial y venosa). Por ello se hace necesaria la realización de un ensayo clínico con un
gran número de pacientes, con la potencia adecuada para establecer pruebas definitivas e impulsar la práctica
clínica posterior. Por otro lado, la atención habitual en los pacientes con la tensión arterial alta (hipertensión) que se
someten a cirugía no cardiaca es prevenir cualquier episodio de hipertensión. Sin embargo, con esta estrategia se
puede producir una bajada de la tensión arterial (hipotensión) que también se asociada con complicaciones
cardiovasculares y aumento de la mortalidad. No hay pruebas definitivas a partir de estudios clínicos sobre cuál es
la estrategia óptima en el manejo de la tensión arterial en el período de la cirugía en pacientes hipertensos. Por ello,
existe la necesidad de comparar una estrategia de evitación de la hipotensión con una estrategia de evitación de la
hipertensión en pacientes sometidos a cirugía no cardiaca para determinar el impacto en las principales
complicaciones cardiovasculares y la muerte.

Por todo ello, proponemos la realización de un ensayo clínico aleatorizado (distribuye al azar los tratamientos)
internacional (POISE-3) de ATX comparado con placebo (sustancia sin efecto), y además, evaluaremos una
estrategia para evitar la hipotensión versus una estrategia para evitar la hipertensión en pacientes que se someten a
cirugía no cardíaca.
Objetivo Principal
Determinar si el ATX es superior al placebo en la aparición de una amenaza para la vida, sangrado de órgano
mayor y crítico, y no inferior al placebo en la ocurrencia de eventos trombóticos arteriales y venosos principales; y
determinar el impacto de una estrategia de evitación de hipotensión versus una estrategia de evitación de la
hipertensión sobre el riesgo de muerte vascular y eventos vasculares mayores en pacientes que son seguidos
durante 30 días después de una cirugía no cardíaca.
Objetivo Secundario
Determinar el impacto de ATX en los siguientes resultados a los 30 días después de la aleatorización: un riesgo-
beneficio neto en la variable combinada de muerte cardiovascular y hemorragia no mortal que pone en peligro la
vida y hemorragia mayor no mortal, hemorragia de órgano grave, infarto de miocardio, accidente cerebrovascular,
trombosis arterial periférica y tromboembolismo venoso sintomático; hemorragia mayor según la Sociedad
Internacional de Trombosis y Hemostasia (ISTH); hemorragia que afecta la mortalidad después de cirugía no
cardíaca (BIMS); lesión del miocardio después de cirugía no cardíaca (MINS); e infarto de miocardio.
Determinar el impacto de una estrategia de evitación de hipotensión perioperatoria sobre la mortalidad por todas las
causas; MINS; e infarto de miocardio a los 30 días después de la aleatorización.

Criterios de Inclusión
Los pacientes son elegibles si cumplen todos los siguientes criterios: 1. ¿45 años de edad 2. Que requieran al
menos una admisión hospitalaria durante la noche después de la cirugía no cardíaca 3. Consentimiento informado
por escrito 4. Medición preoperatoria de NT-pro-BNP ¿200 ng / L 5. Si no se dispone de una medición preoperatoria
de NT-pro-BNP, el paciente debe cumplir uno o más de los siguientes criterios: A. Antecedentes de enfermedad
arterial coronaria; B. Antecedentes de enfermedad vascular periférica; C. Antecedentes de accidente
cerebrovascular; D. Sometido a cirugía vascular mayor; E. Cualquiera de tres de los siguientes criterios: someterse
a cirugía mayor (es decir, cirugía mayor ortopédica, intraperitoneal, intratorácica, retroperitoneal ), antecedentes de
insuficiencia cardíaca congestiva, accidente isquémico transitorio, diabetes en tratamiento con un agente
hipoglucemiante oral o insulina, edad> 70 años, hipertensión, creatinina sérica> 175 ¿mol / L (> 2.0 mg / dl),
antecedentes de tabaquismo en los últimos 2 años, que vayan a ser sometidos a una intervención quirúrgica de
urgencias.
Además, los pacientes se consideran elegibles para el ensayo factorial parcial de la presión arterial (BP) si han
estado recibiendo crónicamente ¿1 medicación antihipertensiva.
Criterios de Exclusión
Se excluirán los pacientes que cumplan alguno de los siguientes criterios:
1. Pacientes sometidos a cirugía cardíaca.
2. Pacientes sometidos a neurocirugía craneal.
3. Uso planificado de TXA sistémica durante la cirugía
4. Hipersensibilidad o alergia conocida al TXA.
5. Aclaramiento de creatinina <30 ml / min (ecuación de Cockcroft-Gault) o en diálisis crónica
6. Antecedentes de trastorno convulsivo.
7. Pacientes con ictus reciente, infarto de miocardio, trombosis arterial aguda o tromboembolismo venoso (<3
meses)
8. Pacientes con condiciones fibrinolíticas tras coagulopatía de consumo.
9. Pacientes con hemorragia subaracnoidea en los últimos 30 días.
10. Mujeres en edad fértil que no toman anticonceptivos eficaces, embarazadas o en periodo de lactancia.
11. Paciente ya incluido en el estudio POISE-3
Además, los pacientes serán excluidos para el factorial parcial de la PA si:
1. Pacientes con insuficiencia cardíaca congestiva avanzada (clase funcional III o IV de la New York Heart
Association o fracción de eyección del ventrículo izquierdo ¿30%),
2. Pacientes con aneurisma cerebral no tratado,
3. Pacientes con historia previa de hemorragia cerebral por hipertensión arterial.
4. Pacientes sometidos a cirugía por feocromocitoma o antecedentes de feocromocitoma no tratado,
5. Pacientes que son hemodinámicamente inestables o que requieren vasopresores o apoyo inotrópico antes de
someterse a una cirugía.
6. Pacientes con tirotoxicosis (es decir, hipertiroidismo grave) que requieren tratamiento perioperatorio con
bloqueadores beta.

Scientific Title
PeriOperative ISchemic Evaluation-3 (POISE-3) Trial
Rationale
Bleeding in non-cardiac surgery is strongly associated with an increase in cardiovascular complications and
mortality at 30 days after surgery. Tranexamic acid (TXA) may prevent postoperative bleeding. It has been proven
that intravenous administration decreases bleeding and transfusion in orthopedic surgery; however, there are few
studies in patients undergoing another type of non-cardiac surgery. In addition, its safety has not yet been
established in the context of non-cardiac surgery (ie, its effect on arterial and venous thrombosis). Therefore, it is
necessary to carry out a clinical trial with a large number of patients, with the appropriate power to establish definitive
tests and promote the subsequent clinical practice.
On the other hand, the usual care in patients that have high blood pressure (hypertensive patients) undergoing non-
cardiac surgery is to prevent any episode of hypertension. However, this strategy can produce a diminution of blood
pressure (hypotension) that is also associated with cardiovascular complications and increased mortality.
There is no definitive evidence from clinical trials on what is the optimal strategy in the management of blood
pressure in the surgery period in hypertensive patients. Therefore, there is a need to compare a strategy to avoid
hypotension with a strategy to avoid hypertension in patients undergoing non-cardiac surgery to determine the
impact on the main cardiovascular complications and death.

Therefore, we propose the realization of an international randomized (the distribution of treatment is at random)
clinical trial (POISE-3) of TXA controlled with placebo (harmless substance), and we will evaluate a strategy to avoid
hypotension versus a strategy to avoid hypertension in patients who undergo non-cardiac surgery.
Main Objective
To determine if TXA is superior to placebo for the occurrence of life-threatening, major, and critical organ bleeding,
and non-inferior to placebo for the occurrence of major arterial and venous thrombotic events; and to determine the
impact of a hypotension-avoidance strategy versus a hypertension-avoidance strategy on the risk of vascular death
and major vascular events in patients who are followed for 30 days after noncardiac surgery.
Primary Endpoints
The co-primary efficacy outcome for TXA trial is a composite of life-threatening bleeding, major bleeding, and critical
organ bleeding at 30 days after randomization. The co-primary safety outcome for TXA trial is a composite of
myocardial infarction, non-hemorrhagic stroke, peripheral arterial thrombosis, and symptomatic proximal venous
thromboembolism at 30 days after randomization. The primary outcome for the BP management trial is a composite
of vascular death, and non-fatal myocardial infarction, stroke, and cardiac arrest at 30 days after randomization.
Secondary Objective
To determine the impact of TXA on the following outcomes at 30 days after randomization: a net risk-benefit
outcome as a composite of vascular death, and nonfatal life-threatening bleeding, and nonfatal major bleeding,
critical organ bleeding, myocardial infarction, stroke, peripheral arterial thrombosis, and symptomatic venous
thromboembolism; International Society on Thrombosis and Haemostasis (ISTH) major bleeding; bleeding impacting
mortality after noncardiac surgery (BIMS); myocardial injury after noncardiac surgery (MINS); and myocardial
infarction. To determine the impact of a perioperative hypotension-avoidance strategy on all-cause mortality; MINS;
and myocardial infarction at 30 days after randomization.
Secondary Endpoints
The secondary outcomes for TXA trial are: 1) a net risk-benefit outcome as a composite of vascular death, and non-
fatal life-threatening, major or critical organ bleeding, myocardial infarction, stroke, peripheral arterial thrombosis, and
symptomatic proximal venous thromboembolism at 30 days after randomization; 2) International Society on
Thrombosis and Haemostasis (ISTH) major bleeding; 3) BIMS; 4) MINS; and 5) myocardial infarction at 30 days after
randomization.
The secondary outcomes for the BP management factorial are: 1) all-cause mortality at 30 days after randomization;
2) MINS; and 3) myocardial infarction at 30 days after randomization.
Inclusion criteria
Patients are eligible if they fulfill all of the following criteria: 1. ¿45 years of age; 2. expected to require at least an

overnight hospital admission after noncardiac surgery; 3. provide written informed consent to participate in POISE-3;
AND 4. have a preoperative NT-pro-BNP measurement ¿200 ng/L; OR 5. if a preoperative NT-pro-BNP
measurement is not available, then the patient must fulfill ¿1 of the following 5 criteria: A. history of coronary artery
disease; B. history of peripheral arterial disease; C. history of stroke; D. undergoing major vascular surgery; E. any 3
of the following 9 criteria: undergoing major surgery (i.e. intraperitoneal, intrathoracic, retroperitoneal, or major
orthopedic surgery), history of congestive heart failure, transient ischemic attack, diabetic and currently taking an oral
hypoglycemic agent or insulin, age >70 years, hypertension, serum creatinine >175 µmol/L (>2.0 mg/dl), history of
smoking within 2 years of surgery, undergoing urgent/emergent surgery. Additionally, patients will be considered
eligible for the BP partial factorial if they have been chronically receiving ¿1 antihypertensive medication.
Exclusion criteria
We will exclude patients meeting any of the following criteria: 1. Patients undergoing cardiac surgery
2. Patients undergoing cranial neurosurgery 3. Planned use of systemic TXA during surgery
4. Hypersensitivity or known allergy to TXA 5. Creatinine clearance <30 mL/min (Cockcroft-Gault equation) or on
chronic dialysis
6. History of seizure disorder
7. Patients with recent stroke, myocardial infarction, acute arterial thrombosis or venous thromboembolism (<3
months)
8. Patients with fibrinolytic conditions following consumption coagulopathy
9. Patients with subarachnoid hemorrhage within the past 30 days
10. Women of childbearing potential who are not taking effective contraception, pregnant or breast-feeding
11. Previously enrolled in POISE-3 Trial Additionally, patients will be excluded for the BP partial factorial if:
1. Patients with advanced congestive heart failure (New York Heart Association functional class III or IV or left
ventricular ejection fraction ¿30%),
2. Patients with untreated brain aneurysm, 3. Patients with previous history of hypertensive related cerebral
hemorrhage,
4. Patients undergoing surgery for pheochromocytoma or history of untreated pheochromocytoma,
5. Patients who are hemodynamically unstable or requiring vasopressors or inotropic support before undergoing
surgery. 6. Patients with thyrotoxicosis (i.e., severe hyperthyroidism) requiring perioperative beta-blocker therapy.
