1.1 Introduccin
En la era del surfactante aun quedan algunos neonatos que no se
pueden ventilar adecuadamente incluso con las sofisticadas
tcnicas de ventilacin convencional. Por lo tanto la insuficiencia
respiratoria sigue siendo una de las mayores causas de mortalidad
neonatal. Intensificar la ventilacin convencional con mayores
frecuencias y presiones en la va area, no lleva ms que a un
aumento de la incidencia de barotrauma. Especialmente las
elevadas fuerzas de cizallamiento que resultan de una elevada
presin de amplitud, que hacen dao al tejido pulmonar. As, tanto
la ECMO como la Ventilacin de alta frecuencia pueden ayudar a
resolver esas situaciones desesperadas.
Desde que la VAFO fue descrita por primera vez por Lunkenheimer a principios de los 70, esta modalidad de ventilacin se ha
desarrollado ampliamente y actualmente es usada en todo el
mundo.
1.2 Definicin
Hay tres caractersticas distintivas de la VAFO: el rango de
frecuencia que oscila de 5 a 50 Hz (300 a 3000 rpm); inspiracin y
espiracin activas; volmenes tidal semejantes al volumen del
espacio muerto. (fig. 1.1)

3 Parmetros caractersticos y
variables de control de la VAF
Tres parmetros determinan la ventilacin oscilatoria (fig 3.1):
Primeramente, hay una presin media de la va area (PMA)
alrededor de la que oscilan las presiones; en segundo lugar, el
volumen oscilatorio, que resulta de presiones oscilantes y que
determina esencialmente la efectividad e ste tipo de ventilacin
mecnica; en tercer lugar la frecuencia oscilatoria que determina
el n de ciclos por unidad de tiempo.

9 Complicaciones, contraindicaciones
y limitaciones
9.1 Complicaciones y efectos secundarios
Irritacin: Inicialmente, los nios presentan irritabilidad,
requiriendo mayor sedacin. Sin embargo, suelen tranquilizarse
segn mejora la hipercapnia.
Secrecin: Con suficiente humidificacin, las secreciones no
obstruyen la va area, sino que ms bien se resuelven mejor. Sin
embargo, incluso pequeas cantidades de secrecin o espuma tras la
administracin de surfactante pueden afectar de manera considerable
la eficacia de VAF. Esto se manifiesta en una disminucin del volumen
oscilatorio (VTAFO) o DCO2 [8, 48, 62, 72, 93].
Hemodinmico: A menudo se observa una ligera disminucin de
la frecuencia cardaca. Este fenmeno as como la frecuentemente
observada apnea refleja, se atribuye a un aumento de la actividad
vagal durante la VAF. Una PMA alta puede comprometer el
retorno venoso al corazn y el gasto cardaco, as como conducir
a un aumento de la resistencia vascular pulmonar. Mejorar el
gasto cardaco y la funcin cardaca antes de iniciar la VAF
pueden ayudar a disminuir estos problemas. A veces se puede
observar edema perifrico [29, 32, 102].
Hemorragia intracraneal: Si la VAF favorece la aparicin de HIC
se ha discutido durante mucho tiempo. La condicin crtica del
paciente en la mayora de los casos, est probablemente
relacionado con este efecto. Estudios ms recientes, en los cuales
la AFO se aplic precozmente, no aportan una mayor incidencia de
esta complicacin en comparacin con ventilacin convencional.
No se pudo observar un aumento de la presin intracraneal [22,
23, 16, 45, 46, 59, 78, 81, 89,105, 108, 111].
Hiperinsuflacin: La hiperinsuflacin pulmonar en las
enfermedades pulmonares obstructivas (ej. en SAM) es la
complicacin ms frecuente y causa de fracaso de la ventilacin
oscilatoria. Aqu, especialmente con frecuencias altas e ndices I:E
inadecuados, puede producirse atrapamiento areo masivo.
Muchos estudios hablan de fugas de aire como complicacin de la
VAF [96, 108, 109]. Por otra parte, estudios ms recientes
describen la VAF como una forma de ventilacin que reduce el
barotrauma y la incidencia de fuga de aire [22, 41, 59, 82].

9.2 Contraindicaciones
La nica contraindicacin relativa conocida hasta el momento es la
obstruccin pulmonar. Puede estar presente en la aspiracin
meconial, pero tambin en la DBP o en la bronquiolitis por VRS.
En nuestra unidad, cuando haba dudas, las pruebas de funcin
pulmonar se mostraron tiles [94, 98] (ver apndice 12.2). Si se
plantea aplicar la VAF a pesar de la existencia de obstruccin
pulmonar, se debe asumir el riesgo de hiperinsuflacin importante
con lo que conlleva. No hay publicaciones que presenten la
hemorragia intracraneal o coagulopata como contraindicaciones
de la VAF.

Prematuro de 25 semanas de edad gestacional, peso al nacer de
720 g. Se administran dos dosis de surfactante. Tras 6 semanas
de ventilacin mecnica: DBP. Alta tras 17 semanas de
tratamiento hospitalario. En la actualidad 5 meses de vida, 3300 g,
recibe nuevamente ventilacin mecnica por insuficiencia
respiratoria debido a una bronquiolitis por VRS. Se inicia
tratamiento con ribavirina.
Rx trax: zonas de atelectasia junto con imgenes de burbujas
grandes e hiperinsuflacin. Debido a empeoramiento progresivo,
se decide aplicar ventilacin oscilatoria, aunque un test de funcin
pulmonar muestra obstruccin perifrica importante(ver figura
12.5).
