Todo aquello procedimiento quirúrgico

con o que se pretende reparar la
membrana timpánica y restablecer
cuando se requiera la continuidad de
la cadena osicular.
 TÉCNICAS PARA LA RECOSTRUCCIÓN SIMPLE DEL SISTEMA
TIMPANOOSICULAR: TIMPANOPLASTIA

Su finalidad es Las técnicas

VÍAS DE ABORDAJE:

En las perforaciones pequeñas es posible el abordaje endomeatal o a través del CAE. Cuando
la perforación es mayor, o se sitúa en cuadrantes anteriores o el cirujano tiene menos
experiencia, la vía retroauricular ofrece mejor campo quirúrgico.
Complicaciones del
tratamiento quirúrgico de las
OMCS
 MATERIALES:

En las pequeñas perforaciones (<2mm), se puede

realizar el cierre con un tapón, constituido por grasa
obtenido previamente del lóbulo de la oreja.

En perforaciones amplias se puede utilizar: fascia

del musculo temporal o pericondrio tomado del
trago.

En cualquier de los injertos elegidos hay dos

posibilidades para su colocación: por debajo de los
restos timpánicos o por encima de ellos.
 TÉCNICA MEDIAL ( UNDERLAY)
• Se coloca el injerto por debajo de los restos
timpánicos. Esta técnica es la empleada con
mayor frecuencia.
• Se puede realizar con anestesia local o
general, según el psiquismo del paciente y la
vía de abordaje.
• Las incisiones de este colgajo se realizan a
unos 5 mm del margen timpánico.
• El injerto de fascia o pericondrio se debe
cortar de tamaño suficiente para cerrar
sobradamente en márgenes la perforación y
se coloca el injerto medial al annulus y al
mango del martillo.
TÉCNICA LATERAL ( OVERLAY)
• Coloca el injerto sobre los restos timpánicos.
• Una vez realizado el colgajo posterior
timpanomeatal se despiteliza toda la
superficie externa del tímpano y márgenes
de perforación, dejando expuesta la capa
fibrosa del tímpano.
• El injerto que se deslizará por debajo del
colgajo se coloca sobre la capa fibrosa
rebasando los límites de la perforación entre
2-3 mm.
 TIMPLANOPLASTIA TIPO II Y III

• Tratan la reparación de la membrana timpánica y de la cadena osicular afectada en grado

variable por anquilosis o interrupción.
• La más utilizada es, sin duda alguna, la de tipo III, que atiende a la membrana timpánica
perforada y las destrucciones del martillo y del yunque, especialmente en su apófisis lenticular
(es la lesión más frecuente en la cadena osicular por OMCS), generalmente con estribo íntegro
o parcialmente destruido y con platina móvil o fija.
Las prótesis sintéticas se denominan PORP (Partial Ossicular Reconstruction Prosthesis) o TORP
(Total Ossicular Reconstruction Prosthesis) en función de la reconstrucción parcial o total de la
cadena que se aborde.

 TIMPANOPLASTIA IV

• Se practican mucho menos que las anteriores.

• No se realiza reconstrucción timpánica u osicular alguna, y tiene como objetivo mejorar
la función del oído medio estableciendo una protección de la ventana redonda para evitar
la competencia entre ésta y la oval, al tiempo que a la platina del estribo móvil se deja en
comunicación con el exterior, a través del conducto auditivo externo.
 TIMPANOPLASTIA V

En el pasado, se realizaba una perforación en el esqueleto del conducto semicircular

horizontal a través del cual las ondas sonoras producían un estímulo que, tras un largo e
inadecuado recorrido, alcanzaba la cóclea. Está en desuso.
