Resumen
Los tratamientos con alineadores invisibles están ganando cada
vez más confianza en el tratamiento de casos complejos, sin
embargo, los cierres de espacios en caso de extracciones tienen
la controversia de que los dientes adyacentes se inclinan hacia el
espacio de extracción, haciendo necesario el uso de mecánicas
auxiliares. Aunque es importante remarcar que el uso de alineadores
es una forma cómoda de tratar a los pacientes con menos efectos
secundarios potenciales que con los aparatos fijos convencionales.
Un movimiento poco predecible sin la utilización de
elementos auxiliares es la mesialización de molares, sin
embargo, el manejo de la secuencia de movimientos con el
uso de anclaje es una mecánica bastante predecible6.
En el presente artículo presentamos un caso de una paciente
femenina de 12 años de edad que acude a consulta por la protrusión
del maxilar superior lo que provocaba una gran inseguridad en
la paciente, llegando a tener miedo a sonreír. La complejidad
del caso la aumentaba la sonrisa Gingival de la paciente, los
padres rechazaron cualquier opción quirúrgica presentada.
Se Plantea un tratamiento con el fin de cumplir las expectativas
de la paciente y de los padres: Conseguir una oclusión estable,
estética y evitar implantes dentales en la posición de las
piezas 15 y 25, ya que presentaba agenesias de 55 y 65.
Se consigue los Objetivos del tratamiento: Alineación y
nivelación, Clase I canina, buena oclusión y estetica.

Abstract
Treatments with invisible orthodontics is gaining more and more
confidence in the treatment of complex cases, however, space
closures in case of extractions have the controversy that the adjacent
teeth lean towards the extraction space, making it necessary to
use auxiliar mechanics. Although it is important to note that the
use of aligners is a comfortable way to treat the patients with fewer
potential side effects than with conventional fixed appliances.
An unpredictable movement without the use of auxiliaries is the
mesialization of molars, however the sequence of movements
with the management of anchorage is a quite predictable.6
In the following article we present the case of a 12 years old female
patient who came for increased protrusion of upper jaw, which
caused great insecurity in the patient, becoming afraid to smile.
The complexity of the case was increasing the gingival smile of
the patient, the parents rejected any surgical option presented.
A treatment is proposed in order to met the expectation of the patient
and the parents: Achieve a stable aesthetic occlusion and avoid
dental implants in the position of 15 and 25 because of agenesis.
Treatment goals are achieved: Alignment and leveling,
Class I canine, good occlusion and esthetic.
