Abstracto
Desde su primera descripción hace aproximadamente un siglo,
nuestra comprensión de los circuitos de aleteo auricular (AFL)
ha evolucionado considerablemente. Un circuito AFL puede
tener manifestaciones electrocardiográficas variables (ECG)
dependiendo de la presencia de lesiones auriculares
preexistentes o sustrato auricular deteriorado. Por el contrario,
diferentes circuitos auriculares (del lado derecho o incluso del
lado izquierdo) que incluyen diferentes mecanismos
(macrorreentrante, microrreentrante o focal) pueden
presentarse con una manifestación de ECG superficial muy
similar. El desarrollo de sistemas eficientes de mapeo
electroanatómico de alta resolución ha mejorado nuestro
conocimiento sobre los mecanismos de AFL, así como facilitado
su tratamiento curativo con ablación con catéter de
radiofrecuencia. Este artículo revisará las características del ECG
para los aleteos típicos y atípicos, y enfatizará las limitaciones
para la ubicación del circuito desde el ECG de superficie.
Introducción Ayuda
El término aleteo apareció por primera vez hace un siglo, en 1887, con
Mac William, quien describió los fenómenos visuales resultantes de la
"estimulación farádica de las aurículas que las convierte en un aleteo

rápido". Fue solo 23 años después que Jolly y Ritchie, utilizando el
modelo de Cambridge del galvanómetro de cuerdas de Einthoven,

registraron el primer ejemplo de ECG de AFL.
Perspectivas clínicas
Si bien existen estudios epidemiológicos detallados de la fibrilación
auricular (FA), estudios similares de AFL son menos frecuentes. El
aleteo auricular ocurre <1/10 con la frecuencia que la FA. Los estudios
de la base de datos del Área de Estudio Epidemiológico de Marshfield
han informado que la incidencia general de AFL es de ∼88 por 100 000
personas-año. La incidencia de AFL en los menores de 50 años es de
∼5/100 000, pero aumenta bruscamente a 587/100 000 en los >80

años.

El aleteo auricular coexiste o precede a la FA. En un estudio

longitudinal, la FA se desarrolló en el 56% de los pacientes con AFL

solitaria. También se ha demostrado que el 25-82% de los pacientes
que se beneficiaron de la ablación exitosa de RF para la AFL típica
aislada experimentarán una nueva aparición de FA durante el

seguimiento. La estrecha interrelación entre AF y AFL es

compleja. Algunos autores han demostrado que los pacientes con FA
coexistente y AFL deberían beneficiarse de la ablación de FA con
catéter de RF, eventualmente asociada con la ablación de AFL en lugar

de la ablación de AFL solamente. Se acepta que los pacientes que
presentan AFL deben beneficiarse de la misma profilaxis
antitrombótica que para la FA, de acuerdo con la presencia de un
riesgo tromboembólico significativo (cha2DS2-VASc score). El
período de periablación para la AFL debe incluir las mismas
precauciones que para la cardioversión electiva para la FA.

Intra-isthmus reentry
This is a recently described entity which may be included in the group
43
of typical AFLs. The surface ECGs show typical CCW pattern in most PDF
