Los pasos bsicos para poder aplicar a una residencia medica en EEUU son estos:
Aplicar para el ECFMG ID USMLE Step 1 USMLE Step 2 CK USMLE Step 2 CS Certificacin por la ECFMG* ( no es necesario tenerla al momento de aplicar a la especialidad pero si para comenzarla)
Comenc a estudiar al comienzo del pre- internado, para el USMLE Step 1, el tiempo para prepararse vara de persona a persona desde promedio de 6 meses a un ao generalmente.
El material para prepararme:
Kaplan Books (el mejor en mi opinin), Serie Rapid Review, Kaplan Qbank, USMLEWorld (mejor banco de preguntas), NMS, Med essentials, First aid Step 1. Puedes saber cmo est tu nivel acadmico para el examen tomando los NBME (National Board of Medical Examiners) self assesments (en mi experiencia son muy precisos tome uno 1 mes antes del examen solamente, y la calificacin fue muy parecida)
Para poder tomar el examen debes hacer un proceso burocrtico en la UASD, el decano debe firmar el formulario 186 que tienes acceso despus que pagas el examen y se debe enviar desde la universidad a la ECFMG.
El examen se toma en el centro Prometric del Dominico/Americano
Para el Step 2 CK:
Lo mismo, Libros de Kaplan, Blueprints, Step-up Medicine, Kaplan QBook, y el usmleworld q bank, First Aid Step 2 CK, Master the Boards Step 2, Boards and Wards. Si todava eres estudiante, te toca el mismo proceso burocrtico otra vez, formulario 186 el decano lo firma y enviarlo a la ECFMG.
Para el Step 2 CS: Buen ingles, buena caligrafa, normas de cortesa, habilidades Clnicas, Buen manejo del tiempo. Material: First aid Step 2 CS y buen compaero de estudio.
Para el Certificado de la ECFMG: Despus de tomar todos los exmenes eres elegible para certificarte, el paso ms difcil en mi experiencia personal. Una vez que te gradas debes enviar una copia de tu Diploma con una traduccin legal a la ECFMG, mas 2 fotos de 2x2, el formulario 344 y 345 (Medical School Release Request)
Es un proceso puramente burocrtico, la ECFMG enva a la universidad el formulario 327 A, por correo lo cual puede tomar meses en llegar y posiblemente nunca llegar, mi consejo saquen una cuenta con FedEx y enven una autorizacin firmada va Fax a la ECFMG para que la utilicen para enviar este formulario a la universidad.
Pasos: 1. El formulario llega a correspondencia en La secretara General (Rectora). 2. De Correspondencia lo envan a registro (pero debera ser enviado al decanato, pero as en la uasd) 3. Debes ir a correspondencia preguntar con que numero de oficio tu formulario fue enviado a registro. 4. Ir a registro con el nmero de oficio al departamento de Secretaria 5. Solicitar un record de notas de uso externo y una legalizacin de titulo personal, para esto debes ir con una copia de tu cedula, una copia del ttulo, una copia de la portada de la tesis, y una cua si tienes 6. Hacer yuca en la uasd hasta que todos tus papeles estn listos 7. Pagar un envo en FedEX 8. Si tienes suerte y el formulario est bien lleno, sin abreviar apellidos, sin firmas no autorizadas, con el diploma entonces es cuestin de esperar. 9. La uasd no nos permite traducir el record as que una vez que el formulario llega y si llega bien debes pagar $220 para la traduccin de tu record.
** Consejo: Dale seguimiento a tu formulario, llama a la ECFMG en cuanto lleguen tus documentos, trata de hablar siempre con el mismo representante, obtn tu cuenta con FedEx, asegurate de que de la universidad manden todo completo y correctamente.
Para Aplicar a la especialidad en EEUU:
1.Debes registrarte en el ERAS 2. Debes tener los siguientes documentos: Deans Letter (suerte con eso), Letters of recomendations (LORs) 3 minimo, Un record de notas personal, Personal Statement.

La verdad es que se han dado muy buenas recomendaciones de parte de todos aquellos que han posteado su experiencia. As que solo tratare de decir lo que no se ha dicho para no abundar ni saturar. Yo estudie tal y como dicen los dems: step 1: kaplan, first aid, med essentials, goljan, USMLE world. (DIT no existia back then y no quise hacer el Kaplan qbank to be able to meet my deadline, aunque lo tena incluido en el calendario) Lessons learned: Pierde el menor tiempo posible en Kaplan, prstale MUCHA ATENCION, pero no lo repitas ni le des larga, porque ah es donde se va el mayor tiempo de la preparacin. Kaplan es para aprender no para memorizar, para eso est el First AID. The more you read the higher the score (thanks Edward F. Goljan) as que si te da tiempo lee to lo que te pase por delante, recuerda que a veces puedes leer mil veces un concepto de un libro, y no entenderlo pero si lo lees de uno que te lo explique mejor lo entiendes a la primera. Muchas preguntas son muy importantes, as que si puedes haz los 2 Bancos de preguntas ms usados (Kaplan q Bank y USMLE world) En mi opinin es mejor hacer el kaplan qbank primero porque se parece menos al examen y te refresca lo aprendido en los libros de kaplan. Sirve ms o menos de transicin hacia el World, y hace que te vaya mejor en el mismo. El USMLE world es IGUALITO al examen, solo que mas difcil. Los porcentajes en estos bancos son muy importantes. En world: Con ms de 50% de seguro que no te quemas ms de 55: es un 80 y pico Mas de 60: 90 y pico en el examen Y con ms de 68-70 es un 99 sin duda. Por eso: Haz tus bancos de preguntas de la manera ms honesta posible: timed, sin libro abierto, sin Tutor Mode, de TODAS LAS MATERIAS mezcladas y sin ningun Sneak peak (estar haciendo preguntas de WORLD mientras lees los libros de kaplan). No le tengas miedo al examen, despus que tus numeritos te den para lo que quieres no pierdas ms tiempo y dale pa alla, recuerda que esto es solo el primer paso. step 2ck: Kaplan books (did not do the videos), First Aid (7th ed! las anteriores son horribles, pero esta es lo mximo), Step up TO MEDICINE, Crush Step 3 (si STEP 3), Boards and boards, USMLE world (ms que suficiente pal step 2)

Como recomendaciones generales puedo decir lo siguiente: 1. El tiempo para los exmenes vara dependiendo de la persona. Usted va a durar lo que usted dure, no se preocupe por eso. Solo tenga presente que se hace en 6 meses a 1 Ano como tiempo promedio, siempre que se le este dedicando el tiempo adecuado. No crea tampoco que estudiando 3 horas al da vas a tomar este examen antes del 2023, porque no lo haras (yo me pase los primeros meses de mi preparacion con este concepto erroneo de que 3-4 horas eran suficientes, porque eso fue lo que me dijeron los gringos en mi internado... how very wrong they were). Con esto del tiempo en mente hago mi segunda recomendacion. 2. Never lose sight of the big picture: El step 1 es el examen mas dificil de todos, el ms largo de preparar, el ms tedioso, el ms frustrante y el mas hijo de la gran $%^ de todos, pero nunca se te olvide que es solamente un examen de 4 exmenes que vas a tener que tomar, mas una Aplicacin que vas a tener que preparar, mas las Entrevistas que vas a tener que hacer. As que no dejes que el step 1 se convierta en una obsesin ni un gran obstculo. Fjate con el pero ponte una meta y ten siempre presente lo siguiente: Lo ideal es tener las 3 notas (step 1, 2ck y 2CS) para el 1ro de septiembre, Octubre como mas tardar. Simplemente porque los hospitales tienen un nmero limitado de entrevistas y no quieren arriesgarse a darle una entrevista a alguien que no saben si va a tener sus 3 exmenes pasados y su ECFMG certificate a la hora del match. Las fechas para el CS son escasas, as que programate con tiempo para pagarlo y poner la fecha. Recuerda que este examen se PREPARA EN 1 MES. 3. La competencia esta dura, con todo el material que hay disponible hoy en da mucha gente saca 99. Pero no te desanimes si no sacas 99, nosotros tenemos muchas herramientas que nos hacen atractivos para muchos programas (el espaol, la personalidad, la cultura, etc). Siempre apunta al 99, pero no te obsesiones con l. 4. Si quieres especialidades muy montras SI SE PUEDE, pero requiere ms sacrificio y el porcentaje de xito es menor, pero SI SE PUEDE. 5. Si quieres una especialidad no muy montra, tampoco creas que son muy fciles de conseguir, asi que no te duermas mucho. 6. USCE USCE USCE. Hoy en da se est haciendo cada vez ms importante tener Experiencia en gringolandia para ser considerado un buen candidato. As que trata de hacerlo antes de aplicar. 7. A los programas les importa muy poco lo que hayas hecho aqu en RD (they dont know what you do here or how you do it!) asi que no tomes trabajos que creas que te van a dar un beneficio a la hora de aplicar pero que te exijan mucho tiempo. Hay que trabajar pa buscarse los chelitos (cosa que tampoco los gringos entienden bien, para ellos estudiar es primero, y el que no tiene $ que pida un prstamo), pero trata de que sea un trabajo cmodo, con las menos horas posibles y el menor estres posible. La pasanta es ms o menos importante, porque se justifica con el hecho de que hay que hacerla para el exequator, pero eso no significa que tengas que hacerla obligatoriamente. 8. Failure in one exam is not the end of the world. Estos exmenes no miden tu capacidad como profesional ni significan que un 99 sea mejor medico que un 76. Lamentablemente son necesarios porque los programas necesitan tener alguna unidad de medida con que compararnos a nosotros los IMGs entre nosotros mismos y con los AMGs. Si te quemas en un examen esto no te hace menos medico que nadie, y muchos program directors lo saben, por lo que siempre te van a dar la oportunidad. Siempre que tengas en tu mente que quemarse es una posibilidad y que en el peor de los casos puede suceder pero no ser el fin del mundo como lo conocemos eso te va a quitar mucho peso de encima y te va a ir mejor. 9. Tu vida cambia significativamente estudiando pa esta vaina. Pero la verdad es que esto simplemente fue un cambio que debi haber sucedido aos antes, cuando entraste a estudiar medicina. Trata de mantener tus hobbies y de darte tu tiempo todos los das y todas las semanas para que puedas mantener la salud mental. Recuerda que un buen grupo de apoyo es importante, gente que est pasando por esto o que ya lo haya pasado. Recuerda que a veces para los muggles (la gente que no estudia medicina) es difcil entender esta vaina, asi que no se sientan mal si sus amigos o hasta sus padres cuestionan lo que ests haciendo y no son de mucha ayuda. 10. Ten tus metas claras, esto puede ser duro y muchas distracciones y "alternativas" pueden surgir, pero mantente firme en lo tuyo. Some of us turn into USMLE zombies, thats ok cus it helps keep everything else away.

Got my score yesterday! Got my score yesterday, exactly the third Wednesday after the Wednesday that followed my exam date (03/22/11), at 10:36 am Easter Time.
I'm very happy with the results 99 (261), and just wanted to passed by and thank everyone in this forum and contribute by posting my study plan and impressions.
My Study Plan was as follows: I studied 8-12 hours everyday (only 4-6 on Sundays) very consistently. My suggestion is take some breaks, including some weekends off when you feel burned out (did this one or twice during my prep). In my opinion, you should stop working if possible. I did overnight shifts two or three times per week which I think possibly affected my performance due to chronic sleep deprivation. But in my case, I was able to study and do a few blocks of questions during work because I worked as a polysomnography technician.
1. Kaplan Lecture Notes and Videos: my recommendation with this is do it as fast as you can. Understand every concept, but you do not need to memorize everything (I didn't), but definitively understand it. Also read Microbiology Made Ridiculously Simple (good images that will stick in your head) at that time along with Kaplan Micro.
2. Kaplan Qbank: did about 25% of it when I was finishing reading Kaplan. In my opinion, not essential but overall good.
3. FA: the highest yield book that I used. 1st read out of 3. Also kind of reviewed Kaplan Lecture Notes again at the same time, to annotate everything I thought was important and lacking in FA (use tiny handwriting, as you will need more space for notes later). The idea is to make FA your primary book. Never opened Kaplan books again.
4. DIT: very good program overall. Great if you need to reinforce your pharmacology and they definitively make you memorize a lot of HY concepts and topics. They make you go through FA entirely, so this counted as the 2nd read for FA.
5. FA: Quickly read first aid again (3rd time) before entering UWorld.
6. USMLE World: best learning tool ever. This must be done in random, timed, non-specific subjects, non-tutorial and with complete 46-Q blocks. Do NOT make the mistake of doing it by subject or tutorial. This is important because the final score is a good predictor of your performance in the real deal. Very similar questions to real test, but way harder. The idea is to get over-prepared, and you will if your scores are good enough.
---- Now here is when everything went wrong for me.... I was supposed to take the exam end of January 2010, just after finishing UWorld. Well guess what? I got my Form 186 rejected twice!!! I had to wait 1 month every time it got rejected and ended up receiving my permit in March. Terrible experience. So my recommendation: apply EARLY, at least 5 months. So I had about two months added to my prep and I used them to review a few additional sources such as High Yield Molecular Biology(just a few topics) and Goljan audios (in my opinion, not that high yield, but if you are deficient in path go ahead and do it). ---- Once I received my permit, I took online NBME 7 and booked my test 10 days later. DO NOT take offline NBME. The key answers are totally wrong. I don't know who did that but they are wrong, so don't make that mistake or you will get confused before the exam, and you DONT want that.
This allowed me time to:
7. FA: last read before exam. Probably what helped me the most as I had all my Kaplan, DIT and Uworld annotations in it. At the same time I annotated every piece of information that I wasn't familiar with in a sheet of paper and reviewed it the day before the exam.
8. Webpath: also very high yield before the exam. Went through all neuroanatomy slides and some electron-microscopy slides.
Summarizing Scores: Qbank: 66%, in random, timed, not subject-wise, only 25% of it. UWorld: 77% in random, timed, not subject-wise, did 100% and reviewed some of the incorrect. UWorld SA1: 254, did it at 50% of the qbank. UWorld SA1: 264, did it at 100% of the qbank. NBME 7: 258 (660), 10 days before the exam.
Day before the test: Reviewed some of my notes. Couldn't stop reading until 8:00 pm. Had a light dinner, watched a movie and took a relaxing tea which I knew would help me relieve the anxiety. Had some trouble falling asleep but eventually did at ~11:00 pm.
Test day: Got there really early to avoid traffic and get a parking space. People at Prometric center were really nice. Blocks went smoothly. Took breaks between every block. If you are addicted to coffee like me, beware that there might not be a coffee machine available. In my case it was broken, so not getting my boost after lunch REALLY affected me in the 5th block. I was very sleepy. Overall, I felt confident. It was NOT EASY, but I felt well prepared. About 80% were easy for me. I marked between 5-8 questions per block that either I wanted to read again or I had no clue (1- 3 per block). Fortunately, I had between 10-20 minutes left in every block. I trained myself during UW for this. It's what I like to call the "30-30" rule: be at question 30 by minute 30. This allowed me plenty of time in the test to not only review those marked questions, but even all questions.
FINAL SCORE: 99 (261)
Final words: exam is very doable. If you follow a similar study plan, which is a working formula, you will get over-prepared for 80% of the exam content, and will feel confident during the test. Get a great study partner (thanks dude!) and be consistent. This has been a life-changing journey for me and I'm sure will be the same for you. GOOD LUCK!
