INTRODUCCIÓN:
Hombre, 36 años, sin comorbilidades. Consultó por lesión en falange distal de dedo medio
mano derecha, con exposición ósea, sin compromiso de lecho ungueal, producida por sierra
eléctrica. Se diagnosticó inicialmente como fractura expuesta Gustilo 2, categorizada
finalmente como LPD Allen 2. El manejo consistió en limpieza, debridamiento, Cefazolina 2
gramos endovenoso, vacuna antitetánica y vendaje semioclusivo por 6 semanas continuas.
El resultado del tratamiento fue restauración completa de piel y fanéreos, sin compromiso
funcional.
DISCUSIÓN Y CONCLUSIÓN:
Las lesiones de punta de dedo tratadas con vendaje semioclusivo tienen un alto rendimiento
en la regeneración epitelial y dérmica, con apariencia y sensibilidad similar a un dedo
normal, descrito en la literatura como superior a tratamientos quirúrgicos. Es un manejo
fácil, barato y sin complicaciones, con un excelente resultado en función, sensibilidad y
capacidad de carga, como se ejemplifica en este caso. Es por esto, la recomendación como
manejo ideal en LPD, relevante para no someter al paciente a un estrés quirúrgico
innecesario. Lo anterior excluye circunstancias donde se requiera manejo quirúrgico
obligatorio, como lesiones por desprendimiento, lesiones del lecho ungueal y fracturas de la
falange distal que requieren fijación.

INTRODUCTION:
CASE PRESENTATION:
Male, 36 years old, with no comorbidities. He consulted for injury to the distal phalanx of the
middle finger of the right hand, with bone exposure, without nail bed involvement, produced
by electric saw. It was initially diagnosed as an exposed fracture Gustilo 2, finally
categorized as fingertip injury Allen 2. Management consisted of cleaning, debridement,
intravenous Cefazolin 2 grams, tetanus vaccine and semi-occlusive bandage for 6
continuous weeks. The result of the treatment was complete restoration of skin and
phanereal, without functional compromise.
Fingertip lesions treated with semi-occlusive dressing have a high yield in epithelial and
dermal regeneration, with appearance and sensitivity similar to a normal finger, described in
the literature as superior to surgical treatments. It is an easy, inexpensive and uncomplicated
management, with an excellent result in function, sensitivity and load capacity, as
exemplified in this case. This is why it is recommended as ideal management in fingertip
injuries, relevant to avoid subjecting the patient to unnecessary surgical stress. The above
excludes circumstances where mandatory surgical management is required, such as
detachment injuries, nail bed injuries and fractures of the distal phalanx requiring fixation.
