INFORMACIÓN PERSONAL
Entiendo que IGT Services and Technologies
Colombia S.A.S. y sus entidades afiliadas y
pertenecientes al grupo económico (Colectivamente
“IGT”) requerirán mi información personal (según se
define a continuación) para diversos fines durante el
transcurso del proceso de selección y de mi empleo y en
ciertas circunstancias, incluso después de la finalización
de mi empleo. Por lo tanto, acepto por este medio
proporcionar a IGT Services and Technologies
Colombia S.A.S. o cualquiera de sus entidades afiliadas
"Información personal / IP" según lo solicitado por IGT
Services and Technologies Colombia S.A.S. o
cualquiera de sus entidades afiliadas.
Por la presente, proporciono mi consentimiento expreso
para la recopilación y el uso de la IP provista por mí, de
acuerdo con los términos de este Formulario de
Consentimiento tal como se establece a continuación.

1. Recolectar y conservar mi IP durante el proceso de
selección, al momento de la contratación, durante el
empleo y al cese del empleo, incluida la información
de identificación personal como nombres, apellidos,
dirección de correo electrónico, información familiar
educativa, financiera, historial de empleo, detalles
de viaje, información de salud, mi huella dactilar etc.
únicamente para fines comerciales legítimos de
IGT.
Manifiesto que me informaron que los datos sobre
mi salud y m huella dactilar pueden llegar a ser
información sensible.
2. Supervisar el correo electrónico asignado por IGT,
el uso de Internet en el lugar de trabajo, incluyendo
ubicación remota/en casa en un escenario de
trabajo remoto/en casa, las llamadas telefónicas y
los registros de ingreso y salida en el portal de IGT
y en las puertas de acceso físico a las
diferentes áreas de la compañía, así como los
registros en vídeo de dichos ingresos y salidas,
garantizando el cumplimiento de las leyes de
protección de datos.
3. Compartir la IP internamente entre las entidades de
IGT o externamente con terceros con quienes IGT
tiene relaciones contractuales con el fin de procesar
salarios, realizar verificaciones de antecedentes,
otras actividades administrativas y

PRIVACY NOTICE /CONSENT FORM FOR USING /
PROCESSING PERSONAL
INFORMATION
I understand that IGT Services and Technologies
Colombia S.A.S. and its affiliated and group entities
(collectively “IGT”) will require my Personal
Information (as defined below) for various purposes
during the course of the recruitment process and my
employment with them and in certain circumstances
even after cessation of my employment with them. In
light of the foregoing, I hereby agree to provide and
shall provide to IGT Services and Technologies
Colombia S.A.S. or any of its affiliated or group
entities, such “Personal Information / PI”) as
requested by IGT Services and Technologies
Colombia S.A.S. or any of its affiliated or group
entities.
I hereby provide my express consent for the collection
and use of the PI provided by me in accordance with
the terms of this Consent Form as provided
hereunder.

Collect and retain the PI, during the
recruitment process, at the time of joining,
during the employment, and cessation of
employment, including personally identifiable
information (such as name, middle name, last
name, email address, family information,
educational information, financial information,
employment history information, travel details,
health information, fingerprint, etc.) for IGT
legitimate business purpose.
I declare that I was informed that my health
information and my fingerprint are considered
sensitive information.
Monitor my IGT email communication, internet
usage at workplace, including remote/home
location in work from remote/home scenario,
telephone calls, and timely physical checks on
the portal and door access, as well as any
video recording of the abovementioned
access, ensuring the compliance with Data
Protection Laws.
Share the PI internally amongst IGT entities or
externally with third parties with whom IGT
has contractual relationships for the purpose
of processing salaries, performing
background checks, other administrative
activities and legitimate business
requirements.

 4. Conservar la IP incluso después del cese del
empleo, si así lo exigen las leyes aplicables.
5. Recolectar, procesar y conservar la IP que tenga el
carácter de sensible incluyendo, pero sin limitarse a
datos biométricos, tipo sanguíneo y RH, entre otros.
Para estos efectos, declaro que he sido informado
del carácter sensible de los datos objeto de
tratamiento y de que no estoy obligado a
suministrarlos.
6. Recolectar, procesar y conservar la IP de niños,
niñas y adolescentes respecto de quienes ejerzo la
representación legal, para los fines asociados a mi
relación laboral con IGT, así como para la realización
de actividades lúdicas en las que podré participar en
calidad de empleado de IGT. Manifiesto que, como
representante legal del menor, éste ejerció su
derecho a ser escuchado, opinión que se valoró para
el otorgamiento de esta autorización.
7. Recolectar, procesar y conservar la IP de terceros
respecto de quienes he obtenido la autorización
previa para la transferencia de su IP a IGT para los
fines asociados a mi relación laboral con IGT.
8. Facilitar de las operaciones diarias de IGT ya sea que
se realicen dentro de IGT o desde una ubicación
remota, incluyendo la ubicación de mi residencia en el
escenario de trabajo desde casa, incluidas, sin
limitación, las funciones administrativas / de viaje
diarias.
9. Facilitar las operaciones de recursos humanos,
incluido, sin limitación, el procesamiento de sueldos
y desprendibles de pago, el procesamiento de
asuntos relacionados con los beneficios para
empleados, la verificación de antecedentes, el
procesamiento de VISA en caso de expatriados.
10. En el material producido por o en nombre de IGT
destinado principalmente para uso interno o
lanzamientos de clientes o en el sitio web de la
empresa o de la empresa o para la circulación entre
personal actual o exempleados.
11. Facilitar servicios médicos para empleados y
seguridad física de oficinas.
12. Facilitar el uso y la personalización de IGT Portal /
sitios web / herramientas, incluyendo, y sin
limitación, abordar cualquier problema técnico y/o
solucionar el mismo.
13. Provisión de requisitos comerciales para IGT
(incluido el intercambio de IP con entidades
globales adscritas a IGT o un tercero por una razón
comercial
válida).

Retain PI even after the cessation of
employment only if it is required under the
applicable laws.
Collect, process and retain the PI that can be
characterized as sensitive data, including but
without limiting to, biometric information, blood
type, and RH, beside others. For this purpose,
I declare that I have been informed of the
sensitive nature of the data that will be subject
of the processing, and that I am not compelled
to provide it.
Collect, process and retain the PI of children
and teenagers regarding which I exercise their
legal representation for the purposes related
to my employment relationship with IGT, as
well as for the execution of activities and
events in which I might participate as
employee of IGT. I declare that, as legal
representative of the minor, he/she exercised
their right to be heard and the opinion was
valued accordingly for this authorization.
Collect, process and retain the PI of third
parties from who I have already obtained their
authorization to transfer their PI to IGT for the
purposes related to my employment
relationship with IGT.
Facilitation of daily operations of IGT whether
carried inside IGT or from a remote location,
including my home location in work from
home scenario, including without limitation
day-to-day administrative/ travel functions.
Facilitation of HR Operations including without
limitation processing of salaries and salary
slips, processing of employee benefit related
matters, background verification, VISA
processing to foreigners.
In material produced by or on behalf of IGT
primarily intended for internal use or client
pitches or display in the office premises or
company website or for circulation amongst
any or all of the following - current and former
staff.
Facilitation of employee medical services and
physical security at office premises.
Facilitating the use and personalization of IGT
Portal /website / tools including without
limitation addressing any technical problems
and fixing the same.
Facilitation of IGT legitimate business
requirement (including sharing PI with cross-
border IGT entities or a third party for a valid
business reason).

 14. Compartir con los clientes la IP para cumplir con los
requisitos contractuales y legítimos del negocio.
15. Prevención de actos ilegales relacionados con su IP.
16. Cumplimiento de las obligaciones legales,
respuesta a citaciones, órdenes judiciales o
procesos judiciales, proporciona información a los
organismos encargados de hacer cumplir la ley o en
relación con
investigaciones sobre asuntos relacionados con la
seguridad pública.
17. Motivos de seguridad al interior de las oficinas de
IGT para lo cual ha decidido implementar un
sistema de video vigilancia.
18 Publicación de mi foto, relacionada con mi empleo o
relacionada con los servicios prestados, a través de
medios físicos o digitales, en IGT o en las
instalaciones / propiedades de sus clientes, tanto
físicas como digitales.
Manifiesto que me informaron que mi foto es un
dato sensible.
19. Grabación, monitoreo, recolección, almacenamiento y
auditoria de mi Audio/video (AV) de manera
continua /a necesidad durante la prestación de mis
servicios a IGT o alguno de sus clientes, dentro de las
instalaciones de la oficina y/o en la cuenta del lugar de
trabajo remoto o en el escenario de trabajo desde
casa. Esto también podría incluir grabación,
monitoreo, recolección, almacenamiento y auditoria de
mis alrededores incluyendo personas naturales,
quienes pueden ser grabadas sin intención durante la
prestación de mis servicios a IGT o a cualquiera de
sus Asociados/ Cliente, dentro de las instalaciones de
la oficina y/o en la cuenta del lugar de trabajo remoto
en un escenario de trabajo desde casa.
Se me ha informado que los usos y propósitos
anteriores son legítimos y que IGT cumplirá con las
Leyes de Protección de Datos aplicables mientras
procesa la IP y será responsable por el
incumplimiento de sus obligaciones conforme a
dicha ley.
Me han informado expresamente y entiendo que
para ejercer cualquiera de mis derechos como
titular de la información, puedo recurrir a lo
siguiente:
1. Puedo presentar una queja ante el Oficial de
Protección de Datos de IGT a través de un correo
electrónico a privacy1@igtsolutions.com o mediante
el mecanismo de 'Informe de Incidentes' disponible
en el portal de IGT.
2. También puedo presentar una queja como tercero

contractual and legitimate business
requirements.
Prevention of illegal acts related to your PI.
Compliance with statutory obligations,
responding to subpoenas, court orders, or
judicial process, provide information to law
enforcement agencies or in connection with
an
investigation on matters related to public
safety.
Security motives at the inside of IGT offices,
for which purpose IGT has decided to
implement a video surveillance system.
Publication of my photo, pertaining to my
employment or relating to my services,
through physical or digital medium, on IGT or
its client’s premises/properties, both physical
or digital.
I declare that I was informed that my health
information can be considered as sensitive
information.
Recording, monitoring, collecting, storing and
auditing of my audio-video (AV) on a
continuous/ad-hoc basis during the course of
rendering of my services to IGT or to any of its
client/customer, within office premises and/or
on account of remote work location or in work
from home scenario. This may also include
recording, monitoring, collecting, storing and
auditing of my surroundings, including natural
persons, who may unintentionally get
recorded during the course of rendering of my
services to IGT or to any of its
client/customer, within office premises and/or
on account of remote work location or in work
from home scenario.
I have been informed that the above uses and
purposes are legitimate and that IGT will comply with
the applicable Data Protection Laws while processing
the PI and shall be liable for breach of its obligations
under the said law.
I have been expressly informed and I understand that
to exercise my right as data subject I have this
information:
1. I can lodge a complaint with IGT Data Protection
Officer via an email to privacy1@igtsolutions.com or
through ‘Incident Reporting’ mechanism available on
IGT internal portal .
2. I can also lodge a complaint as a third party

 He sido informado expresamente y entiendo que
tengo los siguientes derechos con respecto a la IP
que he compartido y que compartiré con IGT:
1. Conocer, actualizar y rectificar su IP frente a IGT,
en su condición de Responsable del Tratamiento.
2. Solicitar prueba de la autorización otorgada a IGT,
en su condición de Responsable del Tratamiento.
3. Ser informado por IGT, previa solicitud del Titular,
respecto del uso que se le ha dado a su IP.
4. Presentar ante la Superintendencia de Industria y
Comercio quejas por infracciones a lo dispuesto en
la Ley 1581 de 2012 y las demás normas que la
modifiquen, adicionen o complementen, una vez
haya agotado el trámite de consulta o reclamo ante
el Responsable del Tratamiento, de conformidad
con la ley y esta Política.
5. Revocar la autorización y/o solicitar la supresión de
la IP cuando en el Tratamiento no se respeten los
principios, derechos y garantías constitucionales y
legales.
6. Acceder en forma gratuita a su IP que hayan sido
objeto de Tratamiento.
De acuerdo con lo anterior, cualquier ejercicio de los
derechos anteriores no será serpa objeto de alguna
acción disciplinaria de parte de IGT.
Entiendo, acepto y reconozco lo siguiente:
1. La IP se recopila para los fines especificados en
esta carta y no se procesará para fines distintos
a los mencionados.
2. El procesamiento de la IP, de conformidad con
las Leyes de protección de datos, para fines de
archivo en el interés público, con fines de
investigación científica o histórica o con fines
estadísticos, no se considerará incompatible
con los fines especificados anteriormente.
3. La IP compartida por mí es precisa y está
actualizada. En caso de cualquier cambio,
informaré de dichos cambios dentro de un
tiempo razonable.
4. El uso del ID de correo electrónico oficial solo
es para fines oficiales y no se utilizará para
intercambiar información personal a menos que
lo solicite IGT.
5. Mantenga el PI de otros empleados con la
seguridad adecuada sin obstaculizar la calidad
a través de la destrucción o pérdida y deberá
cumplir con todas las Leyes de Protección de
Datos aplicables. Cualquier incumplimiento de
lo anterior dará lugar a acciones disciplinarias
que incluyen la terminación inmediata de mi

I have been expressly informed and I understand
that I have the following rights with respect to the PI
that I have shared and will share with IGT:
1. Be informed, update and rectify your PI before
IGT, as data processor.
2. Request any evidence of the consent granted to
IGT, as data processor, to process your PI.
3. Be informed by the Company, prior request made
by the data subject, regarding the use that it is
giving to your PI.
4. Submit any complain or claim before the
Superintendence of Industry and Commerce
regarding the breach of Law 1581 of 2012, and
any other applicable regulation, after undertaking
the consultation and claiming procedure
according with this Policy and the Law.
5. Revoke the authorization granted to IGT for the
processing of PI and/or request the deletion of
any PI when its processing is not made according
with the principles, rights and legal and
constitutional guarantees.
6. Freely access the PI subject of the processing by
IGT.
Pursuant to the above, any exercise of the foregoing
rights
by me will not be reciprocated with any disciplinary action
from IGT.
I understand, agree and acknowledge the
following:
1. PI is collected for purposes specified in this letter
and shall not be processed in a manner that is
incompatible with the said purposes.
2. Processing of the PI, in accordance with Data
Protection Laws, for archiving purposes in the
public interest, scientific or historical research
purposes or statistical purposes, shall not be
considered to be incompatible with the above
specified purposes.
3. PI shared by me is accurate and up to date. In
case of any change, I shall report such changes
within a reasonable time.
4. Use the official Email ID for official purposes only
and shall not use it to exchange any personal
information unless requested by IGT.
5. Hold the PI of other employees with proper
security without hampering the quality through
destruction or loss and shall comply with all
applicable Data Protection Laws. Any non-
compliance of the foregoing will lead to
disciplinary actions including immediate
termination of my employment.

mediante el Portal IGT o por solicitud realizada por

Así mismo, vuelvo a manifestar que me fue informado
que no me encuentro obligado a suministrar a IGT los
datos sensibles o relacionados con menores de edad, y,
que por lo tanto puedo excluirlos del tratamiento, sin
embargo, y según lo establecido en la presente
autorización he decidido autorizar su tratamiento con el
alcance acá definido.
Los términos de éste de consentimiento se considerarán
parte integral de mi contrato de trabajo con IGT y se
leerán como si estuvieran específicamente incorporados
en el mismo.

Hereby I declare that I have reviewed IGT data Privacy
Policy and furthermore, that I have been informed of the
possibility to consult it on the IGT Portal or by request
made to the email privacy1@igtsolutions.com
Additionally, I affirm that I have been informed that I am
not compelled to provide sensitive data or data related to
under aged children and teenagers, and, therefore, it
can be excluded from its treatment, nevertheless,
according with this consent, I have decided to authorize
its treatment under the scope here defined.
The terms of this Consent Form shall be deemed to form
an integral part of my employment agreement with IGT
and shall be read as if they are specifically incorporated
therein.
