INTRODUCCIN
El ictus es una de las principales causas de mortalidad y morbilidad a las que se enfrenta la medicina. Aunque afecta a todos
los estratos de la poblacin, su aparicin se incrementa a partir
de los 55 aos y, en el mundo desarrollado, esta enfermedad
constituye la primera causa de invalidez en adultos y es una de
las que ocasionan mayor mortalidad: alrededor de un 10% de las
muertes en los pases industrializados se deben a enfermedades
vasculares cerebrales. Adems, la demencia subsiguiente al ictus comienza a suponer un problema de salud importante. El
80% de los ictus son isqumicos y, por tanto, derivan de la falta de aporte sanguneo adecuado a un rea cerebral. En condiciones normales, el flujo sanguneo cerebral es de 55 mL/
100 g/min, lo que asegura una liberacin continuada de oxgeno
y glucosa, que sirven como substratos para mantener los potenciales de membrana de las neuronas, as como la sntesis y liberacin de neurotransmisores. Cuando el flujo disminuye a valores de 10 a 15 mL/100 g/min, el dao neuronal puede ser irrever-
sible despus de 30 minutos, mientras que en las regiones que rodean a esta zona, donde la reduccin del flujo es menor (de 15 a
35 mL/100 g/min) [1], el metabolismo celular se detiene, pero la
clula se mantiene viable. Esta regin hipoperfundida se conoce
como zona de penumbra y, en su interior, las clulas sufren una
disminucin de las propiedades de excitabilidad elctrica, sin
que se perturben irreversiblemente los gradientes de iones [2,3].
En la zona de penumbra se preserva el metabolismo energtico
y, por tanto, la neurona no est condenada a la necrosis celular.
Sin embargo, si no se restablece la perfusin sangunea ni se
bloquean los mecanismos excitotxicos, el tejido en penumbra
puede derivar hacia la muerte celular. Es posible visualizar este
proceso mediante tcnicas de imaginologa y su recuperacin o
conversin al infarto se ha documentado en modelos experimentales [4]. Los estudios preclnicos en animales indican que, si el
suministro de sangre no se restaura y el tejido no es protegido
metablicamente en un mximo de seis horas, la regin de penumbra se deteriora, lo que contribuye al crecimiento centrfugo
del infarto [5]. El perodo durante el cual la penumbra se mantiene viable abre una ventana de oportunidades teraputicas que
dependen fundamentalmente de la gravedad de la isquemia, del
flujo sanguneo cerebral proveniente de vasos adyacentes, de la
vulnerabilidad selectiva del tejido neuronal afectado y de factores sistmicos como la glucemia srica, la presin arterial sistmica o la temperatura corporal [6].
Los conocimientos adquiridos en los ltimos aos acerca de
la existencia de un rea de penumbra isqumica en todo ictus y
su posible reversibilidad o, por el contrario, derivacin hacia la
muerte celular hacen entender por qu la farmacologa de la neuroproteccin ha adquirido una gran importancia en el tratamiento de la isquemia cerebral. De hecho, actualmente el esfuerzo
clnico en el ictus isqumico persigue dos objetivos principales:
reabrir las arterias ocluidas, con el consiguiente restablecimiento de la perfusin cerebral, y obtener un efecto neuroprotector
que garantice la viabilidad celular hasta que los mecanismos fi-

como antibitico, ha demostrado ejercer una accin neuroprotectora en modelos de isquemia [38] y modula la transmisin
glutamatrgica [39]. En octubre de 2007 se publicaron los datos
del nico estudio concluido hasta la fecha en el que se demuestra una mejora significativa en las escalas de valoracin de los
Institutos Nacionales de Salud de EE. UU. (NIHSS), escala de
Rankin e ndice de Barthel en los pacientes que recibieron minociclina (200 mg/da durante 5 das), en comparacin con placebo, entre las 6 y 24 horas siguientes al accidente vascular cerebral [40]. Por ltimo, los posibles efectos neuroprotectores de
la minociclina tanto en traumatismos cerebrales [41] como medulares [42] han suscitado que en estos momentos haya en curso al menos un estudio piloto, del que todava no conocemos los
resultados [43].
El clometiazol es un agonista del cido gamma-aminobutrico que tiene la propiedad de incrementar la resistencia a la toxicidad celular inducida por glutamato. El estudio clnico CLASS
(Clomethiazole Acute Stroke Study) no confirm la eficacia de
este frmaco y los siguientes estudios, restringidos a pacientes
con grandes infartos carotdeos, tampoco mostraron beneficio
alguno [44]. Es interesante el hecho de que el clometiazol parece ser seguro en pacientes con infartos hemorrgicos e incluso
con hemorragia cerebral. Como efecto adverso principal destaca la sedacin.
La citicolina, un precursor de la fosfatidilcolina, reduce la
lesin de las membranas neuronales mediante el aumento de
la sntesis de los fosfolpidos y la disminucin de las concentraciones de cidos grasos libres. Diferentes investigaciones, tanto
en animales como en humanos, han mostrado que la citicolina
es un frmaco seguro y, al menos en los estudios iniciales, se sugiri que pudiera ser beneficiosa en las primeras horas de un infarto cerebral. Sin embargo, un estudio ms reciente, que compar 297 pacientes tratados con citicolina en las primeras 24 h
de evolucin con otros 127 que recibieron placebo, no encontr
diferencias ni en los resultados funcionales ni en la mortalidad,
por lo que se concluy que, aunque es segura, no es eficaz. Posteriormente, un estudio post hoc demostr que, en pacientes con
una puntuacin mayor de 8 en la NIHSS, la recuperacin fue favorable cuando se administraba citicolina [45,46]. En la actualidad, un ensayo clnico en fase III, de distribucin aleatoria y
controlado con placebo estudio ICTUS pretende demostrar la
utilidad de la citicolina en el ictus isqumico grave cuando se
administra precozmente.
Por otra parte, hay pruebas de que las ERO participan en los
procesos de isquemia; se ha formulado la hiptesis de que las
propias ERO (generadas cuando se reinstaura el flujo sanguneo en las reas del cerebro infartadas) son las causantes principales del dao originado por la reperfusin. Se han evaluado clnicamente como posibles frmacos neuroprotectores los
compuestos con capacidad de eliminar radicales libres (tirilazad, ebseleno), los quelantes de hierro (deferoxamina) o los
quelantes de radicales libres (NXY-059), as como la albmina
y las estatinas.
El ms estudiado de ellos es el tirilazad; un 21-aminoesteroide o lazaroide, sintetizado a partir de la metilprednisolona, pero
sin las funciones propias de los glucocorticoides o de las hormonas esteroideas. Aunque los estudios de fase III con este frmaco
quedaron suspendidos por la aparicin de efectos adversos, algunos de ellos haban mostrado una ligera reduccin en la mortalidad y una tendencia a una mejor recuperacin a los tres meses de esta complicacin. En el anlisis de los pacientes inclui-

CONCLUSIONES
Con toda seguridad, en los prximos aos la farmacologa ofrecer nuevos compuestos con los que alcanzar una neuroproteccin efectiva tras un accidente vascular cerebral. Es probable
que, como sucede en la terapia recanalizadora, el tiempo desempee un papel fundamental tambin en la neuroproteccin
tras el ictus. Sin embargo, cuando las rutas intracelulares generales se hayan caracterizado y dispongamos de frmacos neuroprotectores concretos, ser posible perfilar la biologa de cada
paciente con la idea de seleccionar el tratamiento que mejor se
adapte a sus requerimientos individuales e incluso a su perfil
temporal. Aunque nadie discute que los frmacos neuroprotectores deben emplearse lo ms tempranamente posible, su utili-
