TRASTORNO DEPRESIVO MAYOR
Vs Placebo
• Metanalisis : Superioridad d=0.32(Sugarman y col 2014)
• Escalas de Hamilton, MADRS, CGI.
Vs ATC
• Montgomery(2001): no diferencia significativa.
• Paroxetina se tolero mejor, menor tasa de interrupción y mayor efecto
sobre ansiedad.
Vs ISRS
• Eficacia = frente a fluoxetina en TDM y ansiedad asociada.
• Mayor tasa de abandono frente a sertralina y escitalopram.
• Escitalopram superior en fase aguda de tratamiento(Purgato y cols 2014)
Vs Otros
• Mirtazapina superior, Raboxetina inferior en respuesta temprana
• Tasas mas altas de remisión con Venlafaxina XR
• Paroxetina superior a antagnositas NK1.(sustancia P)

• 2003: FDA emite declaración sobre aumento de riesgo de pensamientos e intentos suicidas en
niños en tto con paroxetina para TDM(3.4% vs 1.2%) y eficacia limitada(Keller y cols. 2001)
• 2015: beneficios menores y riesgos mayores de los iniciales.(Keller y cols 2015)
• Mayoría de eventos suicidas en los primeros 6 meses con ISRS , sin diferencia significativa en
los distintos ISRS(Schneeweiss y cols. 2010)
• Farmacocinética distinta:
Fobia social
• Sugerida por primera vez en ensayos clínicos abiertos en 1996.
• Paroxetina, sertralina, fluvoxamina CR y venlafaxina XR, únicos aprobados por FDA
• 20-40 mg dosis óptima
TAG
• Primer estudio 1997 lo comparó con imipramina y BDZ., mostrando la superioridad sobre BDZ en HAM-
A
• Dosis habitual eficaz de 20 mg al día.
TOC
• Igual eficacia que clomipramina con mayor tolerabilidad para paroxetina(Zohan 1996)
• Eficacia para TOC pediátrico con buena tolerancia(Geller y cols 2004)
• Generalmente dosis>60 mg/dia Mejoría clínica 10-12 semanas .
OTROS TRASTORNOS
TEPT
• Sertralina y paroxetina aprobadas por FDA
• Mejorías significativas en síntomas de re experimentación, evitación e hiperactivacion
comparado a placebo(Marshal y cols 2001)
• Exposición prolongada es superior para remisión de síntomas(Popiel y cols. 2015)
TDPM
• Fluoxetina, sertralina y paroxetina aprobadas por FDA
• Paroxetina CR mayor eficacia en tratamiento comparado a placebo(Cohen 2004)
• Dosificación en fase lútea y fase continua, superior a placebo para síntomas psicológicos

• Procedimientos para evaluar tendencias suicidas no estandarizados
• Depresión como FR para comportamiento suicida en personas en tratamiento
OTRAS CONSIDERACIONES
Seguridad
• No se asocia con anomalías en pruebas de laboratorio, EEG o ECG
• No disminuye FC como otros IRSN
• Menor riesgo de prolongación QT

EMBARAZO Y LACTANCIA
• Único ISRS en categoría D de embarazo
• Mujeres metabolizadores lentas: mayores niveles de paroxetina y mayor riesgo de teratogenicidad fetal.
• Síntomas recién nacidos expuestos a ISRS: temblor, hipertonicidad, irritabilidad, convulsiones, llanto
débil o ausente y mala alimentación. Se resuelven en 2 semanas
• Orsolini y cols 2015: paroxetina y sertralina con mejor perfil de seguridad en lactancia materna.
