MOTION FOR SPECIAL IMMIGRANT JUVENILE STATUS FINDINGS

Yo, CARMELINA VASQUEZ DEL TRANCITO, la firmante nombrada a continuación, declaro bajo
juramento lo siguiente:

I, CARMELINA VASQUEZ DEL TRANCITO, the undersigned party, declare under oath the following:

1. Yo tengo conocimiento de la Petición que se presentará y de una audiencia sobre la Custodia y

Hallazgo de Hechos para el Estatus Especial de Inmigrante Juvenil para mi hijo legal, HENRY
ORALDO JIMENEZ VASQUEZ, que se llevará a cabo en el Tribunal de Juveniles y
Relaciones Domésticas de Distrito en el Condado de Chesterfield, Virginia, Estados Unidos de
América.

2. I am aware of the Petition that will be filed and that a hearing regarding Custody and
Findings of Fact for Special Immigrant Juvenile Status for my legal son, HENRY ORALDO
JIMENEZ VASQUEZ, that will take place in the Juvenile and Domestic Relations District
Court in the County of Chesterfield, Virginia, United States of America.

3. Yo por este medio, con conocimiento, capacidad mental, libre y voluntariamente, autorizo que
el Tribunal de Juveniles y Relaciones Domésticas de Distrito en el Condado de Chesterfield le
otorgue la custodia completa, legal, y física de mi hijo legal a la Sra. INGRID MARLENY
JIMENEZ VASQUEZ.

4. I hereby knowingly, with mental capacity, freely and voluntarily authorize that the Juvenile
and Domestic Relations District Court in the County of Chesterfield grant sole legal and
physical custody of my legal son to Ms. INGRID MARLENY JIMENEZ VASQUEZ.

5. Yo por este medio, con conocimiento, capacidad mental, libre y voluntariamente, autorizo que
el Tribunal de Juveniles y Relaciones Domésticas de Distrito en el Condado de Chesterfield le
otorgue la Moción en Favor del Estatus Especial para Inmigrantes Juvenil e ingrese un

 Hallazgo de Hechos. Yo no estoy disponible para proteger ni proveerle a mi hijo legal el
cuidado necesario para su bienestar.

6. I hereby knowingly, with mental capacity, freely and voluntarily authorize that Juvenile and
Domestic Relations District Court in the County of Chesterfield grant the Motion in favor of
Special Immigrant Juvenile Status and enter a Finding of Facts. I am unable to protect nor
provide my legal son with the care necessary for his well-being.

7. Consiente de mis derechos, con capacidad mental, libre y voluntariamente renuncio a la

citación oficial y toda otra notificación en este proceso de acuerdo con el Código de Virginia §
8.01-286.1. Entiendo que tengo el derecho de revocar esta renuncia en cualquier momento
antes del otorgamiento de la moción.

8. Being aware of my rights, with mental capacity, freely and voluntarily waive my right to
service and all other notifications in this process pursuant to the Code of Virginia § 8.01-
286.1. I understand that I have the right to revoke this waiver at any time before the motion is
granted.
