Introducción
La creciente demanda de los pacientes por restauraciones estéticas o sin
metal, junto con el interés permanente de la profesión por materiales que
preserven los tejidos, ha derivado en el desarrollo real de restauraciones
adhesivas, estableciendo claramente que es posible un nuevo enfoque
biomimético para la odontología restauradora mediante el uso ordenado
de materiales “parecidos a los dientes” (resinas compuestas y cerámicas)
y la generación de adhesivos a los tejidos duros (esmalte y dentina). Los
estudios científicos y la experiencia clínica han validado el uso de estas
restauraciones estéticas adheridas, y podemos haber entrado en la
denominada "era post-amalgama"1.
La invasión mínima
La preparación de la cavidad convencional, como indicó GV Black, ya no
se considera conservadora. Igualmente los principios generales de forma
de retención y de resistencia son a veces inútiles, debido a la unión
lograda con los protocolos adhesivos biomiméticos que permiten la unión
a las estructuras del diente, imitando al natural.
Conclusiones
1. El siglo actual ha obligado repentinamente a la odontología, a
adoptar un nuevo paradigma en relación con los estándares
esperados para la atención a los pacientes 43.
2. El concepto biomimético se basa en un enfoque conservador, que
ha alcanzado resultados satisfactorios y estéticos.
3. El abrumador peso de la evidencia contemporánea favorece la
odontología mínimamente invasiva (después que la prevención ha
fallado), y una amplia gama de equipos, materiales y técnicas está
disponible para conservar la máxima cantidad de tejido dental sano
/ reparable y permitir una restauración biológicamente aceptable.
4. La misión debe ser preservar la estructura del diente, y restaurar
sólo cuando sea necesario. Incluso cuando se indique una
restauración, la eliminación de los tejidos duros debe ser centrado
en la lesión y el objetivo preservar, en la medida de lo posible la
estructura sana del diente.
