Resumen
Abstract
La leche materna contiene ms de 130 oligosacridos distintos, que constituyen el tercer componente de sta. Su concentracin total disminuye a medida que avanza el curso de lactancia, de modo que al ao contiene menos de la mitad que en
las primeras semanas de vida. Los oligosacridos ms abundantes en la leche humana son libres (no conjugados) y neutros
y contienen fucosa. Los oligosacridos fucosilados pueden tener un enlace 2-fucosa o 3-fucosa, composicin que vara segn la procedencia de las madres y el curso de la lactancia. Los
oligosacridos actan como receptores competitivos sobre la
superficie celular del husped, previniendo as la adherencia
de determinados patgenos. La fraccin de oligosacridos no
digerida en la leche materna estimula el crecimiento de bifidobacterias en el colon, y esta flora podra tener efectos beneficiosos de proteccin frente a infecciones entricas. As, los
oligosacridos son un componente principal del sistema inmunolgico innato por el cual la madre protege a su hijo de patgenos (entricos o de otra localizacin) durante la lactancia.

Introduccin
monosacridos: D-glucosa (Glc), D-galactosa (Gal), N-acetilglucosamina (GlcNAc), L-fucosa (Fuc) y cido silico (Sial). A partir
de la lactosa (extremo reductor) se aaden residuos de Gal con
un enlace b-1-6 o residuos GlcNAc unidos en enlace b-1-3 por
medio de glucosiltransferasas. Las molculas se van haciendo
ms complejas a medida que se aaden residuos de Fuc o lactosamina en diferentes posiciones.
Oligosacridos presentes
en la leche humana
Los OS de la LH son hidratos de carbono que tienen lactosa en
el extremo reductor y fucosa o cido silico en el extremo no
reductor. Aunque pueden contener entre 3 y 32 monmeros, la
mayora de OS de la LH tienen 3-8 molculas de los siguientes
Human milk contains more than 130 different oligosaccharides. Oligosaccharides are the third constituent of human milk.
The total concentration of oligosaccharides decline over the
course of lactation, so that the mean concentration after one
year is less than half that in the first weeks of life. The most
common human milk oligosaccharides are free (non-conjugated), neutral, and containing fucose. Fucosylated oligosaccharides can be 2-linked fucose or 3-linked fucose, and these patterns of milk oligosaccharides vary among individuals and
during lactation. Oligosaccharides act as competitive receptors
on the host cell surface, thereby preventing adhesion of a number of bacterial and viral pathogens. Part of the undigested
oligosaccharides in human milk may contribute to stimulation
of the growth of bifidobacteria in the colon, and this flora might
have beneficial effects such as some protection against enteric
infections. Thus, oligosaccharides are a major constituent of an
innate immune system of human milk, whereby the mother protects her infant from enteric and other pathogens through
breastfeeding.

1.200 OS, ya que permite caracterizar los ismeros.
Sntesis de oligosacridos neutros
en la leche humana
Los OS neutros ms importantes se forman a partir de la lactosa
por medio de glucosiltransferasas (GT), de modo que se forman
3 OS centrales, o core: lactosa, lacto-N-tetraosa (LNT) y lacto-Nneo-tetraosa (LNnT), tal como se observa en la figura 15.
Estas 3 molculas (lactosa, LNT y LNnt) darn lugar a distintos compuestos por fucosilacin mediante la accin de fucosiltransferasas (FUT), de modo que el enlace de fucosa se realiza
en la posicin alfa 1-2 o en otra posicin (sobre todo alfa 1-3).

La adicin de fucosa es dependiente de la accin de, al menos, 3 FUT diferentes (figuras 2 y 3):
FUT-2, enzima controlada por determinantes genticos del gen
Se (secretor) presente en la sangre. Da lugar a OS con enlaces
alfa 1-2 de fucosa (OS-F2). Los ms importantes son:
LNF-I (lacto N-fuco-pentaosa I) o antgeno H1, que se forma
al actuar la FUT-2 sobre LNT.
2-FL (2-fucosil-lactosa) o antgeno H2, sintetizada al actuar
la FUT-2 sobre la LNnt.
La FUT-2 est presente en el 77% de poblacin caucasiana.
FUT-3. Enzima controlada por determinantes genticos del
gen Lewis (Le) de la sangre. Da lugar a OS con enlaces alfa
1-3 de fucosa (OS-F3). Los ms importantes son:
LNF-II (lacto-N-fuco-pentaosa II) o antgeno Lewis a, que se
forma al actuar la FUT-3 sobre LNT.
En un 5% de la poblacin se puede formar LNF-III (lacto-Nfucopentaosa III), o gen Lewis x, y otros compuestos en los
que las enzimas implicadas son FUT-4, FUT-5, FUT-6, FUT-9,
etc., determinadas genticamente por otra familia distinta
del gen Se y del gen Lewis.

Efecto antinfeccioso
En estudios in vitro se ha demostrado que los OS de la LH tienen una estructura semejante a la de los receptores especficos hidrocarbonados de bacterias, toxinas y virus, de modo que
actan como receptores competitivos en las clulas intestinales de superficie del husped, previniendo la adhesin de patgenos13. Adems, hay glucoprotenas y glucolpidos en la
estructura de membrana de las clulas de mamferos que pueden enlazarse a mediadores humorales y participar en la informacin transmembrana14.
Se ha demostrado que los OS-2 fucosa (determinados por el gen
Se) inhiben el enlace con ligandos de clulas del husped con:
Campylobacter jejuni (inhibe la adherencia a las clulas epiteliales)15.
Calicivirus16.
Enterotoxina ST de Escherichia coli17.
