EXPERIENCIA

El trismo es una afección patológica de los músculos de la masticación que comúnmente

afecta a pacientes que se han sometido a procedimientos dentales, cirugía oral y radioterapia
para el cáncer de cabeza y cuello. También se usa el término Apertura Restringida de la Boca
(Restricted mouth Opening RMO). Ambos términos indican dificultad para abrir la boca. El
RMO ocurre en diversas condiciones de la cavidad bucal; en la infección faríngea posterior, que se
denomina trismo reflector; en fibrosis submucosa oral; en trastornos de la mucosa oral; en el uso de
ciertas drogas; y en procedimientos dentales menores de la cavidad bucal posterior.

Paciente masculino de 26 años, acude a la consulta por presentar dolor durante la apertura bucal, la
cual está limitada, luego de someterse a un tratamiento por caries dental en las 3 últimas molares
inferiores derechas, el que fue realizado en un periodo de 5 días teniendo cada tratamiento una
duración de entre 45 minutos a una hora en la que mantenía la boca abierta. Luego de 2 días de
terminado el tratamiento, el paciente manifiesta que empezó a sentir dolor y que disminuyó el rango
de la apertura bucal, asimismo que cuando moviliza la mandíbula hacia el lado izquierdo aumenta el
dolor. Al ser examinado se determinó que la distancia interincisal máxima era de 5 mm (valor normal
40 mm) y usando la escala análoga visual del dolor, indicaba un valor de 6/10 a la apertura bucal.
Asimismo, manifestaba tener muchas dificultades para alimentarse y que solo estaba ingiriendo una
dieta blanda y líquida. El paciente no presentaba antecedentes de problemas en la articulación
temporo mandibular. ¿Porqué siente mayor dolor al hacer movimientos laterales hacia la izquierda?
