inadequate chronic care of these diseases. The present paper questions that a lot of
global efforts aimed at improving the care of chronic diseases still responds to a
vertical logics. On the basis of the evidence provided by the Cuban health system
and that of other countries in Europe and Latin America, this paper puts forward
the strategy of the primary health care as a feasible solution for improved approach
to chronic diseases. The need of working for an international agenda that not only
encourage the strengthening of health systems but also their transformation as well
was stated. Given the magnitude and complexity of challenges ahead for the
contemporary health systems, a triple transformation in the epistemological,
operative and ethical fields is advocated.

Las estrategias de Atención Primaria en Salud (APS) podrían constituir una solución
factible para mejorar el abordaje de las enfermedades crónicas. El Foro Europeo
para la Atención Primaria (The European Forum for primary care) asume que un
sistema de salud fuertemente basado en atención primaria está mejor preparado
para ofrecer cuidados en salud integrales y coordinar efectivamente la atención a
las enfermedades crónicas, que aquellos sistemas de salud con una atención
primaria débil.10 En el mismo sentido, Willison & Andrews11 afirman que la APS
constituye la mejor opción para asegurar la continuidad del cuidado a través de los
sistemas de salud y además la mejor manera de reorientar la profesión médica de
su práctica actual: costosa, enfocada en la enfermedad y centrada en el
tratamiento; hacia un sistema de cuidado primario más costo-efectivo y orientado a
la prevención. Por lo menos tres países Latinoamericanos (Brasil, Costa Rica y

características claves que deben exhibir los sistemas de salud resilientes: garantía
de acceso universal y continuo a los servicios de salud; protección del riesgo
financiero; voluntad política y suficiente inversión; prestación de servicios de
calidad; permanente trabajo intersectorial y movilización social; regulación
efectiva; talento humano bien capacitado, compensado y motivado; adecuada
relación médico-paciente; y participación ciudadana.
