Brief Incident Description

En el área de chancado, el mecánico soldador recibe la orden de

trabajo escrito para instalar 02 piezas grating en el piso del Reductor y motor
reductor y al costado de la faja 11A. El mecánico soldador toma la acoplado a la parte
decisión de instalar 01 sola pieza de grating en el piso en lugar de superior

2 piezas de acuerdo a la orden escrito de trabajo y al percatarse

que el sujetador del reductor estaba sobre el área de trabajo,
entonces el toma la decisión de retirar el pin del sujetador
desbloqueando el reductor utilizando una herramienta manual, en Retiro del pin de
sujetador del
ese instante se libera la energía haciendo que el eje del reductor reductor
y el motor (230Kg) acoplado en la parte superior giren en su
Área de instalación de
dirección e impacte en su hombro y pierna derecha 02 piezas grating en el
piso

Simulación del evento, se aseguró el motor al tecle para la simulación.

Consequences
Diagnóstico: Fractura de húmero, tibia y peroné derecho
Immediate Actions
• El ayudante del soldador reporta el evento a la central de emergencia.
• Se trasladó al mecánico soldador a la Posta Médica de la Unidad para su inmediata evaluación y atención.
• El mecánico soldador es trasladado a una clínica local para su evaluación y atención médica especializada.
• El área del evento quedó bloqueada para el levantamiento de información y evidencias.
• Se conformó un equipo multidisciplinario y se inicia la investigación con la metodología ICAM.
