I hereby declare that all information is true, and that the applicant is an active member of this Church, by which
I authorize, that the Chaplaincy be granted, since it is a brother/sister who has had a personal experience with
Jesus, as Savior and Lord. He is a believer who has the support of his church, is knowledgeable and a doer of
the scriptures, and is willing to serve his brothers unconditionally, reflects the character and purpose of Christ,
lives in the fruit of the Holy Spirit, has his home in order, is meek and humble, has a life of holiness and a life
of prayer.
I hereby declare that all information is true, and that any action and behavior on my part or FALSE
INFORMATION, I release all responsibility, My Pastor and the Association, stating with my signature that
I am responsible for any use of this Chaplaincy, and that of leaving the Ministry, I lose all the privilege and right
of any material and representation.

CLAUSULAS:
a)- Si el Capellan, no esta activo, y nunca reporta actividades,se le suspendera su Credencial y su placa.
b)-La cuota que se da, es para gastos de materias y Maestro, por lo tando,
no es reembolsable, en caso de suspension, por mal comportamiento.
c)- El Capellan tiene que reportarse con su Lider de Zona, para ver las actividades.
d)- El Capellan debe asistir a las actividades que se lleven a cabo dentro de La Asociacion, asi como, seminarios,
conferencias, ensenanzas y mas..Para el crecimiento en su Rama.
e)- Para obtener un Rango en la Capellania, se tomaran en cuenta, toda las actividades y conducta del Capellan,
para con sus Lideres.
f)-El/ nuevo aspirante a la Capellania, presentara una constancia de su records Criminal.

Ser capellan, en este Pais, es un Privilegio, o en cualquier otro, pero tambien, hay responsabilidades
muy importantes como un Ministro de Dios primeramente, y como un buen Capellan ante las Leyes Civiles.

CLAUSES:

a)- If the Chaplain is not active, and never reports activities, you will be suspended your Credential and your
badge.
b) -The quota given, is for expenses of materials and Teacher, therefore, is not refundable, in case of
suspension, for misbehavior.
c)- The Chaplain has to report with his zone leader, to see the activities.
d) - The Chaplain must attend the activities that are carried out within The Association, as well as, Seminars,
conferences, teachings and more ..For growth within your Branch.
e)- To obtain a rank in the Chaplaincy, all the activities, and conduct of the Chaplain, to their Leaders.
f) -The new aspirant to the Chaplaincy, present a record of his criminal records.

To be chaplain, in this Country, is a Privilege, or in any other, but also, there are very important
responsibilities as a Minister of God first, and as a good Chaplain before the Civil Laws.
