PRLOGO
El hgado graso es el detonante de una serie de enfermedades
metablicas y cardiovasculares, su real importancia se est
develando poco a poco mientras ms mdicos y cientficos
investigan sobre el tema.
Su peligrosidad radica en que es muy comn (del 30 al
50% de la poblacin general lo padece) y en que no da sntomas
importantes sino hasta que se complica con otras patologas como
diabetes o enfermedad cardiovascular. Su evolucin hacia cirrosis
es en realidad una mnima parte de su letalidad y ms bien es una
complicacin marginal y poco comn.
Su diagnstico por ultrasonido ha hecho que sea ms
fcil su identificacin y aunque no tiene una sensibilidad del
100% (se ha reportado una sensibilidad del 90%), sin embargo,
es mucho menos peligrosa y costosa que la biopsia (el ndice
de mortalidad por biopsia heptica es del 1 al 2 por 10.000 y el
ndice de complicaciones generales como sangrado o perforacin
del rgano es del 0.3 por ciento (49).
El hgado graso comienza con una resistencia heptica
a la insulina, la cual puede ser diagnosticada con una glucosa
en ayunas con valores mayores a 100 mg/100 ml estado al que
algunos mdicos definen como prediabetes. Puede tambin
sospecharse su existencia en pacientes con hipertrigliceridemia,
obesidad (ndice de Masa Corporal sobre 25), aumento del
dimetro de la cintura, diabticos, en ancianos y en personas
con poca actividad fsica o consumo crnico de anticonceptivos,
antidepresivos, antiinflamatorios, bloqueadores de los canales de
calcio o alcohol.
Tanto el hgado graso como la prediabetes son estados
predictivos de futuras enfermedades cuyo ejemplo ms
significativo es un infarto cardaco. Un destino del que parece
no podemos escapar. Sin embargo, ahora es posible prevenir y
cambiar este final poco deseable.

Existen conceptos errneos sobre el tratamiento y fisiopatologa
del hgado graso que merecen ser aclarados y que se tratan
pormenorizadamente a lo largo de la obra:

LA CLNICA DEL HGADO GRASO
NO ALCOHLICO
El hgado graso es una de las enfermedades de mayor incidencia
en la poblacin dado que una de cada dos personas la padecen.
Solo recientemente se est reconociendo que el hgado
graso es pieza clave en el desarrollo de enfermedad cardiovascular
o diabetes y su contribucin en el establecimiento de estas
enfermedades no puede seguir siendo soslayada. Se tiene el falso
concepto de que el hgado graso solo involucra alteraciones a
nivel heptico, pero esto no es del todo verdad, pues si bien es
cierto que el hgado graso puede complicarse con cirrosis, este
hecho representa un porcentaje mnimo que solo ocurre en un 5%
de casos, por el contrario las complicaciones cardiovasculares o
metablicas derivadas del hgado graso varan entre el 50, 80
100% segn diferentes autores.
El hgado graso no solo se produce por el consumo de
alcohol, sino por una serie de causas como medicamentos,
infecciones, problemas endcrinos, enfermedades autoinmunes,
enfermedades genticas, etc. Pero la causa ms importante
de hgado graso se debe al exceso de ingesta de caloras o
falta de ejercicio que llevan a obesidad intraabdominal. Esta
clase de hgado graso se llama enfermedad de hgado graso no
alcohlico (EHGNA) o NAFLD por sus siglas en ingls (Non-
alcoholic Fatty Liver Disease). De hecho, de cada cinco pacientes
diagnosticados con hgado graso, cuatro estn relacionados con
una dieta hipercalrica y obesidad intraabdominal y solo uno con
ingesta de alcohol (95). En realidad se puede afirmar que, para el
hgado, comer en exceso es tan peligroso como un consumo alto
de alcohol.

Los protones del agua y los lpidos contribuyen a las seales en la resonancia
magntica. Estos constituyentes tienen diferentes frecuencias magnticas. El
vector magntico del agua y grasa se pueden alinear (imagen in phase-IP-) u
oponerse (imagen out of phase-OP-). En NAFLD la imagen OP (1) pierde la
intensidad de la seal (el hgado es ms obscuro) con relacin a la imagen del
hgado (H) IP (2).

Cuando se describe la fisiopatologa del hgado graso y sus
consecuencias siempre se termina mencionando a la resistencia a
la insulina, el sndrome metablico y el tejido adiposo en exceso.
Todas estas entidades estn ntimamente asociadas y se influencian
unas a otras creando crculos viciosos que agravan sus estados
patolgicos. A continuacin se describirn esas relaciones.

La definicin de hgado graso alcohlico naci de la necesidad
de clasificar en un grupo a los pacientes que presentaban daos
histolgicos hepticos exclusivamente por consumo de alcohol
(esteatosis macrovesicular, por ejemplo). Posteriormente,
se encontr que otro grupo de pacientes presentaban iguales
caractersticas histopatolgicas pero no consuman alcohol lo
que defini al hgado graso no alcohlico como una entidad
separada. A continuacin se dar una breve comparacin entre
estas entidades.

PRIMARIO
Muchos investigadores han demostrado a travs de modelos
en animales que una dieta hipercalrica (no necesariamente
hipergrasa) genera NAFLD y resistencia a la insulina. Asimismo,
la influencia de la dieta en el desarrollo del hgado graso primario
se pone de manifiesto en estudios en gemelos monocigticos en
donde un gemelo que llega a ser obeso desarrolla hgado graso
y resistencia a la insulina mientras que el que se mantiene en su
peso normal no lo hace (26, 117). Lo que sugiere que la dieta
hipercalrica causa hgado graso primario independientemente
del fondo gentico debido a que genera resistencia intraheptica
a la insulina (nota: no confundir con la resistencia perifrica a la
insulina).
Cmo sucede esto? Los mecanismos involucrados se
discuten a continuacin.

que controla los genes lipognicos, sino que existe otro que se
llama SREBP (Sterol Regulatory Element Binding Protein) el
cual aumenta sus niveles intracelulares por estmulos tales como
un incremento de insulina o IL-6. En condiciones de resistencia
a la insulina es muy comn que exista hiperinsulinemia
compensatoria, as como aumento de produccin de IL-6 por
parte de los adipocitos viscerales. Estos dos factores juntos
aumentan el RNAm del SREBP y por ende la lipognesis de
novo, agravndose as la esteatosis (138).
El aumento de TGs intracelular incrementa la secrecin de
VLDL por parte del hgado y es por esta razn que en el NAFLD
sea comn encontrar aumento de los niveles sricos de VLDL
(figura # 9 ). El aumento de secrecin de VLDL se da por tres
mecanismos 1) normalmente la insulina reprime la sntesis de
ApoB, la protena principal de las VLDL. En caso de resistencia
heptica a la insulina, se aumenta la sntesis del RNAm de la
ApoB y por tanto existe un aumento de la sntesis de VLDL que
pasa a la circulacin (139); 2) El aumento de TNF- produce
aumento de NF-KB el cual activa tambin la sntesis de ApoB
(140) y 3) la estabilidad y el aumento de la vida media de la ApoB
es regulada en parte por la concentracin de cidos grasos libres
en la clula.
Mientras ms alta sea la concentracin de cidos grasos
ms se aumenta la estabilidad de la ApoB lo que se traduce en
mayor cantidad de VLDL en el hepatocito y por tanto en sangre.
En la resistencia a la insulina heptica hay aumento de cidos
grasos libres y por tanto se aumenta la estabilidad de la ApoB y
por ende su concentracin plasmtica (130). As se explica el por
qu en el NAFLD es comn encontrar hipertrigliceridemia.
Pero el hgado no solamente aumenta su contenido celular

El papel que la resistencia a la insulina juega en la gnesis del
hgado graso y en el desarrollo de prediabetes o diabetes es tan
importante que es necesario hacer una pequea revisin sobre el

en un aminocido tirosina lo que lo activa y se inicia una cascada de seales
que activa una enzima detrs de otra como una hilera de domins. Sin
embargo, este efecto no es tan sencillo pues el receptor de la insulina est
regulado por mltiples factores como la kinasa IKKB que desactiva el
receptor de la insulina en vez de activarlo 1) el TNF- activa la IKKB ya sea
directamente o a travs de la produccin de ceramida. A su vez la IKKB
fosforila el receptor de la insulina en un aminocido incorrecto (la serina) y
lo bloquea; 2) la IL-6 activa el sistema de ubiquinacin que destruye el IRS

El NAFLD no solo se complica con enfermedades hepticas sino
que genera complicaciones en otros rganos y sistemas. Esto se
produce de la siguiente manera:
El hgado graso simple est sometido a dos tipos de noxas
1) una sobrecarga de cidos grasos libres que vienen tanto de
la circulacin portal como de la circulacin general y 2) una
sobrecarga de TNF- e IL-6 que vienen de la circulacin portal
provenientes del tejido adiposo intraabdominal. Juntos estos dos
factores activan las clulas Kupffer que son la mayor fuente de
TNF- del organismo. El TNF-, a su vez pasa a la circulacin
general y produce resistencia sistmica a la insulina.
La resistencia perifrica a la insulina puede ser identificada
fcilmente por el test HOMA-IR y cuando se presenta es el
prembulo para el establecimiento de varios de los componentes
del sndrome metablico as como de cuadros que no se consideran
parte del mismo. Dado que el hgado graso lleva a resistencia
a la insulina y sta se complica con diferentes enfermedades se
puede tambin afirmar que el hgado graso es el copartcipe de las
complicaciones de la resistencia a la insulina.
A continuacin se describirn todas las complicaciones
extrahepticas que la dupleta NAFLD/resistencia a la insulina
producen y que son entre otros: hiperinsulinismo, obesidad,
hipertensin, dislipidemia, diabetes, aterosclerosis, estados

Existe la creencia de que el hgado graso es una enfermedad de
los adultos y que es muy rara encontrarla en nios. Posiblemente
esta percepcin se haya originado porque antes se consideraba
que el hgado graso y la cirrosis se deban en su mayor parte al
consumo de alcohol. Sin embargo, ahora se ha demostrado que el
alcohol no es la causa principal del hgado graso y ms bien se ha
creado desde hace algunos aos atrs la categora de hgado graso
NO alcohlico observndose de que el mismo no es exclusivo de
los adultos.
Efectivamente, el hgado graso no alcohlico tiene una
prevalencia alta en nios. Se ha reportado que entre el 3 al 10%
de la poblacin peditrica tienen NAFLD (40), alcanzando una
prevalencia del 65-70% en nios que tienen sobrepeso (97). En
este ltimo grupo la prevalencia del NASH puede ser del 20% y
del sndrome metablico el 24% (97). En Ecuador se ha informado
que la prevalencia de sobrepeso y obesidad es del 20-30% (98).
Extrapolando el riesgo de padecer hgado graso que tienen los
nios con sobrepeso, se deduce que en el Ecuador existira una
incidencia del 14% de nios con NAFLD. La edad tambin es
importante en la prevalencia de la enfermedad, as, se reporta una
prevalencia del 0.7% en nios menores de 2 aos, pero sube hasta
el 17% en pacientes de 15 a 19 aos (99).
Se considera obesidad infantil, si un nio tiene un peso
sobre el percentil 95 en las curvas antropomtricas de peso
tomando en cuenta la edad y el sexo y se define como sobrepeso
si est entre los percentiles 90 a 95 (100). Sin embargo, debido
a las variaciones de este valor dependiendo del grupo tnico,

Por otro lado, qu medidas tomar si el paciente abandona
el ejercicio o la dieta o estamos ante un paciente artrsico o muy
anciano que sea incapaz de hacer ejercicio?
Ante estos casos se necesita una alternativa teraputica y
aqu es donde cumplen su papel los sensibilizadores de la insulina
o los antioxidantes. La prevencin del NAFLD es importante
pues al hacerlo tambin evitamos que aparezca la resistencia
perifrica a la insulina, la prediabetes, la diabetes y especialmente
las enfermedades cardiovasculares como la hipertensin.
Muchas veces el mdico recomienda calcio para prevenir
la descalcificacin, multivitaminas para la fatiga, vitamina C
para el sistema inmune, como terapias preventivas para varias
enfermedades como la osteoporosis, la anemia o infecciones. Sin
embargo, ninguna de estas enfermedades tiene tantas connotaciones
ni estn relacionadas con una mortalidad elevada como lo est el
hgado graso. De ah que, est plenamente indicado y justificado
dar una terapia preventiva para NAFLD. Ms an en pacientes
peditricos en donde un NAFLD no tratado o no evitado puede
dar origen a un paciente diabtico o hipertenso a la corta edad de
40 aos como se expuso anteriormente en este libro.
Muchos son los factores de riesgo en la produccin de hgado
graso en general, por lo que amerita su profilaxis especialmente
en casos de:

ASOCIACIN DE FRMACOS CONTRA EL
NAFLD Y LA RESISTENCIA A LA INSULINA
Innumerables estudios clnicos reportan que los sensibilizadores de
la insulina o los antioxidantes, administrados independientemente
a las dosis adecuadas son un tratamiento eficaz en el NAFLD y
la resistencia a la insulina y queda implcito que la unin de los
dos agentes tendr acciones sinrgicas en el tratamiento de estas
entidades. En el captulo 13 se presentan los resultados de los
principales estudios clnicos que confirman este concepto y las
dosis a las que obtuvieron estos efectos. Muchos de estos estudios
revelan que los antioxidantes a dosis adecuadas pueden revertir la
disfuncin endotelial y tener ciertos efectos antihipertensivos.
Uno de los medicamentos que tiene la unin de la
silimarina y coenzima Q10 a las dosis adecuadas para poder
obtener resultados clnicos satisfactorios es el Kufer Q Recargado.
Este medicamento a dosis de 2 cpsulas por da proporciona sobre
los 300 mg de silimarina que es la dosis mnima que se necesita
para observar efectos teraputicos en el NAFLD. Igualmente a
esta dosificacin proporciona las dosis mnimas de coenzima
Q10 (60 mg) con las que se ha visto disminucin del estrs
oxidativo, apoptosis celular y mejora en la disfuncin endotelial
(ver captulo 13 sobre estudios clnicos para mayor informacin
al respecto).
Por lo tanto es importante describir ms ampliamente este
producto.

A nivel extra-heptico:
Mejora la resistencia a la insulina
Reduce los niveles de hiperinsulinismo cuando que sean
secundarios a una resistencia a la insulina. Por lo tanto el
Kufer Q recargado es til tambin en pacientes con sndrome
de ovario poliqustico en caso de que esta enfermedad est
acompaada de hiperinsulinismo (50 a 60% de los casos) o
en casos de obesidad acompaada de hiperinsulinismo
En pacientes con sndrome metablico y resistencia
a la insulina, mejora los parmetros de los factores de
riesgo cardiovascular como la resistencia a la insulina, la
disfuncin endotelial, la dislipidemia y la tolerancia a la
glucosa.

KUFER Q FORTE
Es otra presentacin de Kufer Q (silimarina 140 mg, coenzima
Q10, 10 mg), su menor dosificacin puede ser utilizada como
profilaxis del hgado graso o despus del consumo de alcohol o
comidas hipercalricas.
Los pacientes en riesgo de desarrollar NAFLD son los candidatos
para la administracin de este producto (obesos, alcohlicos,
fumadores crnicos, pacientes geritricos, etc)
KUFER Q KIDS
Es una presentacin en tabletas efervescentes para nios. Cada
tableta efervescente que se puede diluir en medio vaso de agua (o
tomarla directamente en la boca) contiene 100 mg de silimarina
y 10 mg de coenzima Q10. La dosis es de 1 tableta x da en nios

CMO DIAGNOTICAR NAFLD SI ES UNA
ENFERMEDAD ASINTOMTICA?.
El NAFLD es tan comn que se reportan incidencias del 33% al
46% en la poblacin general o hasta del 58% en hispanos (40,
498). Por lo tanto, diagnosticar y tratar NAFLD es una tarea de los
mdicos de todas las especialidades (inclusive pediatras) en vista
del peligro que encierra esta entidad para la salud del paciente y
las complicaciones hepticas y cardiovasculares a las que lleva.
Dado que el NAFLD es asintomtico, se debe
primariamente sospechar su existencia. Para ello nos ayuda un
examen general que incluye evaluar los siguientes datos: medidas
antropomtricas (peso, estatura y permetro de la cintura), presin
arterial, un examen de sangre donde se medirn glucosa, insulina,
triglicridos, HDL y creatinina; y un examen de orina para medir
microalbuminuria
Con el peso y la estatura podemos calcular el ndice de masa
corporal (IMC), segn la frmula del apndice 5. El IMC nos
ayuda a diagnosticar sobrepeso si ste es mayor a 25.
La tabla del apndice 1 nos da los valores normales
de IMC para nios segn la edad. Por sobre el percentil 85 se
diagnostica sobrepeso. El apndice 2 nos presenta una tabla de
IMC para adultos.
El permetro de la cintura nos da el diagnstico de obesidad
abdominal si su valor est sobre los 88 cm en mujeres o sobre 102
cm en hombres.
Si existe sobrepeso u obesidad abdominal el paciente
tiene un 70 a 80% de probabilidades de tener NAFLD.
Con la glucosa en ayuno entre 100 a 125 mg/dl podemos
descubrir resistencia heptica a la insulina o prediabetes. Con

Caspasas:Son enzimas proteolticas que al activarse destruyen
el ADN y los componentes celulares produciendo apoptosis.
ChREBP: Carbohydrate responsive element binding protein:
es un factor de transcripcin que activa la sntesis de los genes
involucrados en la sntesis de grasas como la sintetasa de cidos
grasos. La activacin de este factor se da por aumento de la
glucosa que se transforma a xilulosa 5 fosfato (una de las vas del
catabolismo de la glucosa), la cual a su vez activa una fosfatasa
(PP2A) que activa el ChREBP.
CYP 2E1: citocromo P450-2E1: Es una enzima involucrada en el
metabolismo de cidos grasos, etanol y xenobiticos. Cataliza la
oxidacin de sustancias orgnicas.

FOXO-1: Forhead Box protein 1: Es un factor de transcripcin
que controla los genes involucrados en la gluconeognesis. La
insulina lo inhibe y por tanto reduce la gluconeognesis.

crnica.
HOMA-TEST: Homeostatic Model Assesment: es una prueba
para medir resistencia perifrica a la insulina.
HSC: Hepatic stellate cell: es un tipo especial de clula en el
hgado. Tcnicamente es un percito. Cuando se activa por varios
estmulos produce colgeno y fibrosis en el hgado.
HSL: Hormone sensitive lipase: es una enzima que produce la
liplisis de los triglicridos.

ICAM: Intercellular adhesion molecule: es una glicoprotena de
la membrana celular del endotelio que funciona como molcula
de adhesin donde se adhieren los leucocitos o plaquetas.
IGF-1: Insulin like growth factor: es una hormona tambin
conocida como somatomedina C. Su estructura es parecida a
la insulina y tiene efectos anablicos. Es importante para el
crecimiento de los nios.
IKK: Inhibitor Kappa Kinase Beta. Es una kinasa de los
aminocidos serina/treonina. Una de sus funciones es controlar
la actividad de la NF-K. Pero cuando se sobreactiva bloquea el

IMC: ndice de masa corporal: Una medida para diagnosticar

IL-6: Interleukina-6: es una citoquina, su principal funcin es

IR: Resistencia a la insulina.
IRS: Insulin Receptor Substrate: es una protena (adaptadora)
que se une al receptor de la insulina y sirve como puente entre
ste y otras enzimas que son posteriormente activadas. Hay dos
subtipos el IRS1 y el IRS2.
IRS-1: Insulin receptor substrate -1. Es la primera protena que
el receptor de la insulina fosforila para iniciar la transduccin de
seales de este receptor.

JNK: JNC Kinase. Es una fosforilasa (kinasa) de aminocidos
serina. Su activacin responde a diferentes seales de estrs y
entre sus funciones estn inducir la apoptosis.

Kupffer: Son macrfagos tisulares hepticos.

MAPKs: Mitogen activated protein kinase: son un grupo de
kinasas (fosforilasas) de los aminocidos serina/treonina que

hgado graso de causas no alcohlicas.
NASH: Non alcoholic steatohepatitis: un estado inflamatorio del

NF-KB: Nuclear factor Kappa-Beta. Es un factor de
transcripcin que activa ms de 100 genes entre los que destacan
genes inflamatorios como la COX2 o genes que controlan la
supervivencia celular.

PAI: Plasminogen activator inhibitor: Una protena de fase aguda
que al aumentar inhibe al inhibidor del plasmingeno por lo tanto
la sangre adquiere tendencia a formar cogulos.
PCR: Protena C reactiva: Una protena que se eleva en casos de
inflamacin crnica o aguda.
PDK: Phosphatidil dependent kinase: es una kinase importante
para la activacin de otras kinasas como AKT o PKC.
PDX: Pancreatic and duodenal homeobox: Es el factor de
transripcin que activa el gene de la insulina y es responsable de
la maduracin de las clulas beta.
PI3K: Phosphatidil inositol 3 phosphate kinase: es una kinasa
que es parte de la cadena de transduccin de seales del receptor
de la insulin.
PKC: Protein kinase C: es una kinasa que fosforila los
aminocidos serina y treonina. Controla varias vas metablicas.
Su sobreactivacin puede bloquear el receptor de la insulina.
PPAR: Peroxisome proliferator activated receptor: es un factor
de transcripcin que controla el metabolismo de los adipocitos y
los lpidos.

reguladora que al unirse al receptor de la insulina o leptina bloquea
sus seales de transduccin.
SOD: Superoxido dismutase: una enzima antioxidante que

TBARS: Thiobarbituric Acid Reactans: es una prueba que mide
la concentracin de perxidos lipdicos que a su vez son una
