Por favor responda a las preguntas en este formulario en base a la conducta típica de su niño/a durante el mes
pasado. Use la siguiente escala de evaluación:
Nunca: la conducta nunca o casi nunca ocurre Frecuentemente: la conducta ocurre bastante seguido
Ocasionalmente: la conducta ocurre algunas veces Siempre: la conducta siempre o casi siempre ocurre
Haga un círculo alrededor de la opción que mejor describa la frecuencia de esta conducta. Trate en lo posible
de contestar todas las preguntas.
Varias preguntas piden saber si su niño/a demuestra “angustia” en ciertas situaciones. Demostraciones de
angustia pueden incluir expresiones verbales (quejas, llantos, gritos) o no verbales (apartarse, gesticular,
empujar algo con fuerza, escaparse corriendo, hacer gestos de dolor, golpear).
Sírvase usar el espacio indicado para añadir comentarios acerca de la conducta o funcionamiento de su niño/a.
