Manejo de artritis reumatoide
• El manejo ha cambiado en los últimos 30 años
• El tratamiento era iniciado tardíamente en curso de la enfermedad.
• Nuevo concepto de artritis temprana.
• Números agentes
• Objetivos del manejo:
• Alcanzar la remisión dentro de los 6 meses
• Al menos un 50% de mejoría clínica dentro de los 3 meses
• Modificar la enfermedad esta por encima del manejo sintomático.
• Desventajas: Alto costo/ riesgo de infecciones

PRINCIPIOS GENERALES
1. El tratamiento de los pacientes con AR, debe tener como objetivo la
mejor atención y debe basarse en una decisión compartida entre el
paciente y el reumatólogo.
2. Las decisiones del tratamiento dependen de la actividad de la
enfermedad y en otros factores del paciente.
3. Los reumatólogos son los especialistas quienes deben manejar a los
pacientes con AR.
4. El reumatólogo debe considerar el alto costo individual, médico y
social de la AR.

RECOMENDACIONES
1. El tratamiento con FAME deben iniciarse lo antes posible.
2. El tratamiento debe buscar la remisión o la menor actividad posible.
3. El monitoreo de la actividad debe ser cada 1 a 3 meses.
4. MTX debe ser parte de la 1era estrategia terapéutica.
5. Si MTX esta contraindicado o no es tolerado, considerar a SSZ O LFN
como 1era estrategia terapéutica.
6. Gc deben indicarse el menor tiempo posible y siempre titular la
menor dosis necesaria.

HIDROXICLOROQUINA
• Antimalárico con propiedades inmunorreguladoras y
antiinflamatorias.
• Presentación: comprimidos de 200mg
• Dosis: Personas con más de 80kg, 400mg/d; menor a ese peso
calcular dosis 5mg/kg/d
• Absorción intestinal aprox. 70%
• Tiempo de vida media 1 a 2 meses. Distribución tisular importante.
• Evaluación oftalmológica anual / retinopatía
• Metabolismo hepático y excreción renal.

• PERFIL HEPÁTICO

Ejercicios
• Paciente mujer de 30 años quien presenta dolor de manos y
muñecas, asociado a rigidez matutina desde hace 2 meses. Al examen
físico presenta sinovitis en articulaciones metacarpofalángicas
proximales y en carpo.
• Qué exámenes solicita para completar su plan diagnóstico?
• Qué diagnósticos diferenciales plantea?
• Conoce alguna herramienta para evaluar severidad y con qué frecuencia
valora respuesta.
• Qué estrategia terapéutica plantea?
2do caso
• Paciente mujer de 55 años con antecedente de artritis reumatoide,
DM2 y ERC. Para la AR RECIBE tratamiento con metrotexate
20mg/semanal y prednisona de 15mg/dia desde aprox hace 4meses.
Acude a consulta para su control. Se evidencia sinovitis. En rx de
mano se evidencia erosiones interfalángicas. En su score DAS28 se
obtiene actividad moderada.
