ABSTRACT
Antihistamines have been used for 50 years becoming one of the most prescribed drugs in Mexico. Current denomination is Histamine Receptor Antagonists and their main task is to avoid the afferent effect of histamine in many
different body tissues. Histamine or -aminoethylimidazole was isolated for the first time by Windaus and Vogt in
1907. In 1910 Daley and Laidlow studied its biological effect and found out that histamine stimulated diverse smooth
muscles. In 1940 the first H1 antagonist antihistamine drug for use in human beings was developed: Antergan
(fenobenzamine) with good results. In 1944 Neo-Antergan (Pyrilamine maleate) was commercialized in 1946, and
further in 1946 diphenhydramine and tripelennamine and in 1949 clorpheniramine. All of these antihistamines was
denominated as First Generation, Classic, or Sedating. In the last 15 years antihistamines with high inhibitory
potential have been developed. Those are designated as Second Generation antihistamines. With the arrival of
new research techniques, new histamine receptors were discovered: in 1966 H1 type, in 1972 H2 type, in 1983 H3
type and finally in 2000 H4 type. After releasing from mast cells, basophiles, histaminergic neurons or other any other cell type, histamine has to join to its receptor: H1, H2, H3 or H4 type. In the central nervous system H3 type is
predominating, allowing self-regulated histamine production as neurotransmitter. Thus wakefulness along with as
appetite, sleep and thermoregulation, are intensified by H1 type receptor. Today H4 type receptor is under study, and
it is know that its function is regulated by inflammatory cytokine production, Therefore its participation in inflammatory processes is assumed. Conclusion: Actually a single pill is enough to block and control the pathologic effects
of histamine, without undesirable secondary effects for the patient, giving him a normal quality of life.

INTRODUCCIN
Los antihistamnicos se han usado durante los ltimos 50 aos para tratar enfermedades alrgicas,
convirtindose en los medicamentos de mayor
prescripcin en Mxico.
Su denominacin actual es Antagonistas de los
receptores de la histamina H1. Su accin consiste
en evitar el efecto aferente de la histamina en los diferentes tejidos del cuerpo, por medio de una competencia y bloqueo en el receptor especfico de la
histamina.
HISTORIA DE LA HISTAMINA
Y LOS ANTIHISTAMNICOS
La histamina o -aminoetilimidazol fue aislada por
vez primera en 1907 por Windaus y Vogt. En l910,
Daley y Laidlow estudiaron su efecto biolgico y descubrieron que estimulaban a diversos msculos lisos, adems de tener intenso efecto vasodepresor.
En 1927, Best y colaboradores aislaron la histamina a partir de muestras frescas de hgado y
pulmn, advirtiendo que dicha amina es constitutiva natural del organismo, acundose el nombre
de histamina con base a la raz griega histos
que significa tejido. 1-3
En 1940, se desarroll el primer antihistamnico
H1 para uso en el humano: Antergan (fenobenzamina) con buenos resultados. 1,2
En l944, se comercializa el neo-antergan (maleato de pirilamina); en l946, la difenhidramina y tripelenamina; y en l949, la clorfeniramina. Todos
estos antihistamnicos H1, por ser los primeros,
fueron denominados de primera generacin, clsicos o sedantes.1,2,4
Durante los ltimos 15 aos se han sintetizado antihistamnicos con alto potencial inhibitorio. A stos
se les denomina antihistamnicos H1 de segunda
generacin y son: loratadina, astemizol, cetirizina,
terfenadina y fexofenadina, entre otros.5
El advenimiento de nuevas tcnicas de investigacin hizo posible el descubrimiento de que la histamina mediaba sus efectos a travs de cuatro receptores, que progresivamente se fueron descubriendo: el receptor H1 en 1966,6 el receptor H2 en l972,7
el receptor H3 en l983,8 y el receptor H4 en 2000.9,10
Los antihistamnicos H1 de primera y segunda generaciones antagonizan al receptor H1. Los antagonistas del receptor H2 se utilizan en la clnica para
reducir la secrecin de cido gstrico en el tratamiento del paciente con enfermedad cido ppti-

Previene la
broncoconstriccin
excesiva, inhibe la
secrecin de cido
gstrico, vasodilatacin
de vasos cerebrales,
funciona como
feedback () para:
liberacin de
neurotransmisores en
los nervios perifricos,
controla la produccin
de histamina en
neuronas
histaminrgicas del
sistema nervioso
central, controla la
liberacin de
neurotransmisores en el
sistema nervioso
central.
Presumiblemente
participa en procesos
inflamatorios como la
alergia y el asma.
su propia produccin de histamina como neurotransmisor. De esta forma, se intensifica el estado de vigilia, por medio del receptor H1, adems del sueo, el
apetito y la termorregulacin.4,15-18
El receptor H3 tambin se encuentra en otros
tejidos perifricos, incluyendo corazn, vasos
(vena safena), tejido linfoide (adenoides), aparato
digestivo y va area; en esta ltima se ha encontrado en los nervios colinrgicos posganglionares
y su funcin es la de mediar la transmisin colinrgica en el bronquio humano, evitando un efecto de exceso de broncoconstriccin. 15
Actualmente, el receptor H4 se encuentra an
en estudio, pero se sabe que su funcin est regulada por la produccin de citocinas inflamatorias, por lo que participan en los procesos inflamatorios. 13,19,20
