INCIDENT DETAILS MANAGERS – KEY POINTS TO CHECK
An employee was operating a front end loader in the coal shed. For unknown
reasons he left the loader and became engulfed in the coal pile near the grizzly bar  Raw material and fuel stockpile storage areas are inspected regularly and

material can unexpectedly slip away.
 The employee, who had approximately 1 year experience in the plant, was working
on the night shift and was operating the front end loader to move coal onto the
grizzly in the coal shed prior to the incident.
 There was significant inventory of coal in the storage building and limited separation
between the stockpiles and the feed pile on the grizzly bar structure.
