frecuencia

The use of operative ultrasound as an aid to liver resection in patients with hepatocellular
carcinoma.

386 patients intraoperative ultrasonography. (245 patients (HCC), and 152 patients tumors less than 5 cm in
diameter. In these 152 patients, intraoperative sonography detected 198 of a total of 203 small HCC's
(sensitivity: 99%), whereas the sensitivity of preoperative ultrasound, angiography, and computed tomography
was 89.3%, 84.1%, and 89.6%, respectively.
Intraoperative ultrasound had slightly lower specificity in differentiating malignant nodules from benign
lesions.
Intraoperative ultrasonography was indispensable in liver surgery, not only for diagnosis of the tumor
spread in the liver, but also as a direct guide for hepatectomy
The use of operative ultrasound in surgery of primary liver tumors
Henri Bismuth, Denis Castaing, O. James Garden
World Journal of Surgery 09/1987; 11(5):610-614
Operative ultrasonography provides more detailed information about liver tumors and their topography
than can be obtained by intensive preoperative investigations
. This information may avoid unnecessary trial dissection of apparently operable tumors and enables more
precise and limited resection of those lesions shown to be resectable by operative ultrasonography
Comparisons of dynamic infusion and delayed computed tomography, intraoperative
ultrasound, and palpation in the diagnosis of liver metastases.
IOUS failed to identify surface lesions less than 1.0 cm in diameter (sensitivity: 40%). Conversely,
palpation was limited in the detection of subsurface tumors less than 1.0 cm in diameter
(sensitivity: 33%). Combined IOUS and palpation were significantly more accurate in the detection
of hepatic metastases than any single modality that was evaluated
Value of Visual Inspection, Bimanual Palpation, and Intraoperative Ultrasonography During Hepatic Resection for
Liver Metastases of Colorectal Carcinoma

Visual inspection, palpation, and IOUS had equally indispensable roles in detecting new nodules during
hepatectomy. Detection was common and usually necessitated alteration, albeit moderately, of the surgical
plan.

we can affirm that liver resection is an imaging-guided procedure and as every interventional
imaging-guided procedure, its features are the highest therapeutic efficacy combined with the
minimal invasiveness. Then, with the intraoperative ultrasonography guidance liver resection
remains the treatment of choice of hepatocellular carcinoma
In conclusion, IOUS accuracy is improved by CE-IOUS with an impact on surgical strategy either for
primary than for metastatic tumors. Furthermore, a wider experience with vascular enhancement patterns
at CE-IOUS could provide new classification for liver lesions

the role of intraoperative ultrasound (IOUS) in intrahepatic staging and the impact on surgical strategy for
patients with colorectal liver metastases (CRLM). The study included 515 patients (1370 MTS)
Performance of CT, MRI, FDG-PET, and intraoperative staging was compared: sensitivity was
63.6, 68.8, 53.6, and 92 % and specificity was 91, 92.3, 95.8, and 97.8 %, respectively
CONCLUSIONS: The use of IOUS continues to be mandatory for correct staging of patients
with CRLM undergoing liver resection. (IOUSCE)

Currently, IOUS is considered an indispensable operative procedure for
intraoperative decision-making and guidance of surgical procedures. For
better surgical practice, education of surgeons in the use of ultrasound is the
most important issue.
