CERTIFICADO DE ASISTENCIA EN VIAJES

Europ Assistance S.A. certifica que la persona identificada como beneficiario en el presente documento ha contratado
un plan integral de asistencia al viajero, con cobertura de asistencia médica por enfermedad y accidentes de acuerdo
con las prestaciones y límites establecidos en cada producto. Los servicios de asistencia en viajes son válidos en
cualquier destino internacional o nacional según plan contratado.

estime conveniente y como comprobante de la cobertura prestada por Europ
Assistance.

Como solicitar Asistencia:

Para activar los Servicios usted debe llamar por cobro revertido vía operadora internacional a alguno de nuestros
números de asistencia 24 horas:

Si en el lugar donde usted se encuentra no es admitido el sistema de cobro revertido vía operadora, comuníquese de
manera directa y luego a su regreso a Chile, solicite el reintegro de los gastos telefónicos incurridos contra la
prestación de los respectivos comprobantes originales.

También puede solicitar asistencia escribiendo un email a: asistencias@europ-assistance.cl

NOTA: El servicio no contempla reintegro de gastos si usted no da aviso a la central de asistencia.

Declaración y Aceptación:
El Beneficiario declara aceptar las Condiciones Generales de los planes de asistencia en viaje de Europ Assistance S.A.
y las prestaciones particulares aplicables al plan contratado las cuales le han sido informadas. El Beneficiario
manifiesta expresamente que conoce y comprende: a) Las exclusiones del servicio de asistencia contratado; b) El
procedimiento a seguir en caso de requerir asistencia en viaje; y c) Sus derechos y obligaciones como beneficiario de
los servicios

travel assistance with health care coverage for illness and accidents according to the benefits and limits of each
product. Services of the travel assistance are valid in any international or national destination according to the plan
subscribed. Products for Europe have benefits and limits which meet Schengen Treaty´s requirements

How to apply for insurance:

To activate the Services you must call collect via international operator to one of our 24 hours
emergency numbers:

If you cannot make a reverse charge call, call us directly and then ask for the reimbursement of the telephone
expenses, presenting the respective original bills.

You can also request assistance writing an email to: asistencias@europ-assistance.cl

NOTE: The service does not provide the reimbursement of expenses if you fail to give not ice to the operational
centre.

Acceptance of terms and conditions:

The holder hereby accepts the general terms and conditions of the travel insurance plan provided by Europ
Assistance SA. This acceptance implies the knowledge and understanding of:a) exclusions from the subscribed
insurance contract b) how to proceed in case of need of health assistance, and c) rights and obligations as holder of
this insurance.
