¿Cómo la diagnosticamos?
Con mucha frecuencia en la IT no hay síntomas y predominan los de las
enfermedades asociadas. La IT grave puede ser bien tolerada durante un largo
periodo de tiempo. De existir, la clínica habitual es de congestión venosa
sistémica (hepatomegalia, ascitis, derrame pleural, edemas) y de reducción de
GC. En pacientes con hipertensión pulmonar e IT disminuyen los síntomas de
congestión pulmonar, pero aumenta la ICD. Es habitual la fibrilación
auricular.
La sustitución valvular tiene una mortalidad operatoria del 7-40% según las
series. La supervivencia a 10 años llega al 30-50%, siendo factores predictores
la clase funcional, la función del VI y del VD y las complicaciones protésicas.
La experiencia actual favorece el uso de bioprótesis sobre válvulas mecánicas.
