Mediante la presente, Como SUPERVISOR y encargado del personal trabajador, en este caso el
operador de maquinaria (EXCAVADORA) el Sr. Daniel Huerta Herrera con C.I.:785881-SC y el
ayudante de la maquinaria (EXCAVADORA) el Sr. Jorge Mendez Vivanco con C.I.:8106171-SC, hago
llegar un informe general de los trabajadores desde 15 de septiembre del 2020 al 30 de
septiembre del 2020, para su correspondiente pago del mes de septiembre.

Además, se adjunta los respaldos correspondientes de los frentes de trabajo, parte mensual del
supervisor, parte diario del operador y el reporte fotográfico.
