• Coartación aórtica.
• Intoxicación por cocaína.
• Displasia fibromuscular.
• Sífilis.
• Enfermedad de Takayasu: los aneurismas de aorta, además de
disecciones, ocurren en le 30% de los pacientes; fundamentalmente se
localizan a nivel de la raíz aórtica y en el 10-20% de los casos se aso-
cian con insuficiencia valvular. Ueda u cols dividen la disección aórtica
que puede tener lugar en la enfermedad de Takayasu en tres variantes
fundamentales:
I. Cuando afecta el cayado aórtico.
II. Afecta aorta descendente y abdominal.
III. Se trata de una combinación de las dos anteriores.
En la necropsia, y en la mayoría de los casos, es posible identificar
una pequeña rotura, que a modo de puerta de entrada, afecta tanto a la
íntima como a la media, facilitando el paso de la sangre. Clínicamente, sin
embargo, esta circunstancia, esta identificación de la rotura, no siempre
tiene lugar. En la mayoría de los casos en los que se intenta documentar
la presencia de la rotura de la íntima mediante el empleo de técnicas de
diagnóstico por la imagen, no se logra.
El lugar donde tiene lugar esta rotura inicial se suele situar un poco
distal, aproximadamente a 2-3 cm de la raíz aórtica, por encima del
ostium coronario derecho, afectando, generalmente, a menos del 50% de
la circunferencia total de la arteria. Es posible, incluso, identificar múl-
tiples roturas, en la mayoría de las ocasiones producidas en diferentes
estadíos evolutivos de la enfermedad.
Su morfología es frecuentemente transversa, aunque también es posi-
ble observar roturas irregulares, diagonales e incluso, longitudinales.
Se han descrito «restitutio» cicatrizales de las roturas, de tal forma que
su evolución natural a la disección no se cumple.
Clínicamente en el 70-90% de los casos existe dolor severo (se llega
a describir «como una puñalada»), que emigra a lo largo de la línea de
disección.
Semiológicamente, su localización generalmente coincide con el lugar
de la disección. Así:
– Dolor torácico anterior: disección de aorta ascendente (tipo II).

En el 95% de los casos en los que sucede una muerte súbita relacio-
nada con la disección aórtica, ésta se asocia con rotura de la falsa luz
creada, mientras que en el 5% restante no es posible demostrar morfoló-
gicamente una causa concreta.
Finalmente cabe señalar que la disección de la aorta abdominal
es rara; aproximadamente representa el 1% del total de las disecciones.
Los sujetos afectados en las diversas series publicadas tienen una edad
media de 62 años. La rotura de la falsa luz, que tiene lugar en el 50% de
los casos, es como causa de muerte una complicación más rara que el
caso de la disección aórtica torácica.
