INTRODUCCIN
Los factores de riesgo para su aparicin son la edad, siendo muy poco
frecuente por debajo de los 30 aos y con una prevalencia mxima entre los 40 y
50 aos, la hipertensin arterial, la obesidad, y el uso de tamoxifeno.
HISTORIA NATURAL
En el nico estudio tipo I con este objeto, 150 mujeres con plipo
endometrial fueron divididas en reseccin histeroscpica versus observacin
durante seis meses, no encontrndose diferencias en el volumen de flujo
menstrual entre los dos grupos, sin embargo el sangrado intermenstrual si mejor
significativamente en el grupo de polipectoma5. As pues, aunque son numerosos
los trabajos publicados que sugieren una mejora de los sntomas en la mayora
de las mujeres a corto y medio plazo, lo cierto, es que son pocos los estudios con
calidad suficiente, lo que limita la solidez de las recomendaciones que pueden
hacerse para la prctica clnica.
Los plipos endometriales tienen una alta prevalencia entre las mujeres
estriles (15,6%-32%), pudiendo ser la causa de la misma y de aborto de
repeticin, y en este caso, la restauracin de una cavidad normal es
indispensable. El mecanismo por el cual pueden dificultar la gestacin y su
evolucin posterior no se conoce. Podran afectar al desarrollo del endometrio al
provocar sangrados o propiciar una zona poco apropiada para la implantacin,
junto con una disminucin en la receptividad endometrial8. Adems se ha
encontrado en estas pacientes un incremento de los niveles de glicodelina,
protena que inhibe la unin entre el vulo y el espermatozoide.
