Presentacin de un caso
Una mujer de 32 aos consulta a su mdico de cabecera porque desde hace 1 mes siente dolor en las manos y los pies y mayor cansancio, los que
atribuye al parto y la atencin de su hijo de 6 meses. Presenta rigidez matinal de 1 hora de duracin y siente alivio al tomar ibuprofeno. Tiene
antecedentes personales importantes como psoriasis, infecciones recientes o diarrea. Se observa tumefaccin en las muecas y sensibilidad a la
compresin de sus articulaciones metatarsofalngicas.
Evolucin
Teniendo en cuenta las caractersticas clnicas de la paciente presentada, su mdico de cabecera la deriv con urgencia al reumatlogo, a pesar de
tener el hemograma, la ESG, la protena C reactiva y el factor reumatoideo normales como as las radiografas de las manos y los pies. Se le hicieron
nuevos estudios que dieron anormales, entre ellos los anticuerpos anti-CCP positivos (>600 U/ml) y la presencia de signos de sinovitis activa sin
erosin en la ecografa de alta resolucin de las muecas con Doppler color. Una opcin til para identificar las lesiones articulares (sinovitis.
anormalidad de los tejidos blandos, edema seo y erosin articular) muchos meses antes de la aparicin de los signos radiogrficos, es la
resonancia magntica. Otras pruebas adicionales realizadas por el reumatlogo llevaron al diagnstico de AR. Como la paciente no estaba
amamantando ni tena planes de embarazo, se inici el tratamiento con frmacos antirreumticos modificadores de la enfermedad, de conformidad
con las recomendaciones para Inglaterra, Gales, e Irlanda del Norte de la National Institute for Health and Clinical Excellence (NICE).8 La AR afecta a
29 mujeres y 18 hombres/100.000 personas al ao, con un nmero similar de pacientes en Suecia y el Reino Unido. Se desconoce la causa de la

enfermedad pero se consideran importantes los factores genticos y ambientales. El inicio suele ser gradual, pero debe diferenciarse de las
enfermedades autolimitadas lo antes posible, para proceder al tratamiento de los pacientes con alto riesgo de dao articular, dada la creciente
evidencia de que la intervencin teraputica precoz mejora el control y reduce el dao. Tambin es importante diferenciar la AR de las artropatas
autolimitadas para evitar el uso innecesario de medicamentos potencialmente txicos.
Los criterios formales de diagnstico de AR no son aplicables a la enfermedad en su forma temprana y no se pueden utilizar para tomar decisiones
teraputicas. Sin embargo, los estudios prospectivos han identificado otros factores asociados con resultados desfavorables que pueden ser
utilizados para apoyar la necesidad de derivacin urgente a un reumatlogo para el tratamiento con modificadores de la enfermedad, siguiendo las
guas NICE, europeas y estadounidenses. El factor de necrosis anti-tumoral y otros agentes biolgicos han hecho posible el tratamiento de la AR con
tanta eficacia que el objetivo es alcanzar una actividad baja de la enfermedad en 3 a 6 meses en la mayora de los pacientes.
