Important information about Permanent (Absentee) Vote-by-Mail Ballot

If you are a registered voter in Contra Costa County, California Law allows you to apply for Permanent Vote-by-Mail status.
Once your application is processed, we will mail you an official ballot for every election in which you are entitled to vote. (E.C.
3201, E.C. 3203)
If you fail to vote in two consecutive statewide general elections, your Vote-by-Mail status will be canceled. You will remain as
an active registered voter and will be notified if you need to re-apply. (E.C. 3206)

This application is for personal use only; its use is not intended for groups or individuals distributing permanent Vote-by-Mail
applications.
